Introduction: Thyroid nodules have become a common finding. Since a small percentage are malignant thyroid neoplasms, it is vital to build a classification system that allows physicians to decide the best approach for each lesion. Thyroid Imaging Reporting and Data System (TI-RADS) was created to take on this challenge. Objectives: Describe TI-RADS from a practical perspective, analyze its main contributes, expose some potential flaws, and display the main procedures that aim to optimize TI-RADS application. Results: TI-RADS addresses 6 features in the thyroid nodules to plan on the need for fine-needle aspiration and follow-up periodicity. Despite being used in several centers, important flaws in TI-RADS are pointed out when compared to other guidelines. To enhance TI-RADS, some authors suggest a combination with other techniques, namely the use of contrast, elastography and a semiautomatic artificial intelligence system. Conclusion: Considering the data available in the literature and despite the doubt raised by some authors about the benefits it may bring in relation to the previous guidelines, it is accepted that TI RADS is useful in the approach of patients with thyroid nodules, allowing a systematic and objective method of evaluating these lesions. It seems to significantly reduce the amount of cytology per fine-needle aspiration, which brings benefits in terms of cost for a health system. Even so, being relatively recent, the cautious application seems to be pertinent, requiring comparative studies between TI-RADS and other thyroid nodule classification systems.
