Abstract: Introdução: Refugiados, deslocados internos e requerentes de asilo partem de países onde existe guerra e violência organizada, enfrentam o perigo do deslocamento e sofrem dificuldades várias no processo de reassentamento. Esta exposição prolongada a adversidade, confere-lhes um risco acrescido de doença mental, cuja prevalência é substancialmente superior à da população em geral, sendo que não existe ainda evidência consensual sobre quais as intervenções mais adequadas no tratamento de perturbações mentais nestas populações. Objectivo: O objectivo deste trabalho é fornecer uma revisão atualizada dos desafios presentes no panorama actual da saúde mental de populações de refugiados, deslocados internos e requerentes de asilo, compreender o contexto histórico, político e social que condicionou o presente enquadramento, analisar a prevalência de doença mental nestas populações e rever as intervenções médicas, psicológicas e psicossociais actualmente utilizadas no seu tratamento. Métodos: A pesquisa foi realizada através das plataformas NCBI, PubMed e ScienceDirect, nas bases de dados MEDLINE, EMBASE, Web of Science, COCHRANE, SciELO, Index e ACAAP e foi restringida a revisões sistemáticas publicadas desde 2015 acerca da prevalência de doença mental e das intervenções terapêuticas em populações de refugiados, deslocados internos ou requerentes de asilo. Resultados: Foram encontradas taxas de doença mental substancialmente superiores às da população em geral e foram relatados os factores condicionantes dessa prevalência. Foi encontrada evidência a favor de abordagens psicoterapêuticas com foco no trauma, abordagens psicossociais e formatos de psicoterapia curtos e não especializados, em países em desenvolvimento. Não existiu evidência consensual para formas de Terapia Cognitivo-comportamental. Conclusão: Apesar de existir uma base de eficácia para determinadas psicoterapias, é necessária mais investigação que confira uma maior evidência em relação aos valores de prevalência e seus factores condicionadores e em relação à eficácia de intervenções terapêuticas especificamente adequadas às necessidades particulares destas populações.
Introduction: Refugees, internally displaced persons and asylum seekers flee countries where there is war and organized violence, face the danger of displacement and suffer various difficulties in the resettlement process. This prolonged exposure to adversity results in an increased risk of mental disorders, for which prevalence is substantially higher than that of the general population, and there is still no common ground of evidence on which interventions are the most appropriate in the treatment of mental illness in these populations. Aims: The aims of this project are to provide an updated review of the challenges present in the current field of mental health in populations of refugees, internally displaced persons and asylum seekers, to understand the historical, political and social context that conditioned the present framework, to analyse the prevalence of mental illness in these populations and to review the medical, psychological and psychosocial interventions currently applied in their treatment. Methods: The research was carried out through the NCBI, PubMed and ScienceDirect platforms, in the MEDLINE, EMBASE, Web of Science, COCHRANE, SciELO, Index and ACAAP databases and was restricted to systematic reviews published since 2015 on the prevalence of mental illness and therapeutic interventions in populations of refugees, internally displaced persons or asylum seekers. Results: Rates of mental disorders were found to be substantially higher than those of the general population and the conditioning factors for their prevalence were reported. Evidence was found in favour of trauma-focused psychotherapies, psychosocial interventions and short, non-specialized forms of psychotherapy, in developing countries. There was no consensual evidence for the use of forms of Cognitive-behavioral Therapy. Conclusions: Although there is a base of evidence for the benefit of certain psychotherapies, more research is needed to provide stronger evidence on the prevalence of mental illness and its conditioning factors and in relation to the effectiveness of therapeutic interventions specifically suited to the particular needs of these populations.
