Abstract: A asma é uma doença inflamatória crónica que ocorre devido a uma hiperresponsividade das vias aéreas podendo obstruí-las e originar uma lesão usualmente reversível. As pessoas que padecem desta doença apresentam tipicamente episódios agudos designados por exacerbações. Estes caracterizam-se por dispneia, tosse, aperto torácico e perturbações a dormir, sendo estes mais frequentes de manhã ou à noite. Estes episódios podem ser de tal modo debilitantes que podem mesmo causar a morte.Esta doença é extremamente prevalente existindo em cerca de 300 milhões de pessoas em todo o mundo, havendo uma maior predisposição nos países desenvolvidos. A taxa de mortalidade desta patologia tem vindo a diminuir desde de o início do século XXI, sendo um fator de risco a má adesão terapêutica. A percentagem de doentes que apresenta a forma mais grave é reduzida e ainda mais reduzida é a percentagem dos que não apresentam a patologia controlada. Contudo esses constituem a maioria dos gastos para o sistema nacional de saúde no âmbito da asma.Estão a ser desenvolvidas novas terapêuticas com o intuito de melhorar o controlo da asma não só porque esta patologia é extremamente prevalente como já foi dito, como também há muitos casos em que o controlo pretendido não é atingido. Em muitos dos doentes em que se tal se verifica deve-se à má adesão terapêutica e à forma incorreta como usam o inalador. As novas terapêuticas são cada vez mais dirigidas como é o caso dos alvos biológicos e imunoterapêuticos sendo nessas novas estratégias que este artigo de revisão se irá focar essencialmente.
Asthma is a chronic inflammatory disease that occurs due to hyperresponsiveness of the airways and it can obstruct them and cause an injury that is usually reversible. People who suffer from this disease typically experience acute episodes called exacerbations. This disease is characterized by dyspnea, cough, chest tightness and sleep disturbances and is most frequent in the morning or at night. These episodes can be so debilitating that they can even cause death.This disease is extremely prevalente existing in about 300 million people worldwide, with a greater predisposition in developed countries. The mortality rate of this pathology has been decreasing since the beginning of the 21st century, with poor therapeutic adherence being a risk factor. The percentage of patients with the most severe form is reduced and even more low is the percentage of those who do not have the pathology in a controlled stage. However, these constitute the majority of expenses for the national health system in the context of asthma. New therapies are being developed with the aim of improving asthma control not only because this pathology is extremely prevalent as already mentioned, but there are also many cases in which the desired control is not achieved. In many the patients in whom this occurs, it is due to poor therapeutic adherence and the incorrect way in which they use the inhaler. New therapies that are being studied have the aim to be are increasingly trying to be more specific, as is the case with biological and immunotherapeutic targets, and it is in these new strategies that this review article will focus on.
