Title: Quantificação da carga anticolinérgica como preditor de resultados clínicos negativos no idoso – um contributo para a prática clínica

Abstract: O envelhecimento da população é uma realidade que caracteriza as sociedades atuais. Os idosos são os principais consumidores de medicamentos, utilizando, muitas vezes, um elevado número de fármacos simultaneamente. A utilização de medicamentos está associada a potenciais danos iatrogénicos graves, situação particularmente relevante na população idosa, em que as alterações fisiológicas do envelhecimento condicionam uma maior suscetibilidade ao aparecimento de efeitos adversos. A distinção entre polimedicação apropriada, terapeuticamente benéfica, e polimedicação inapropriada, potencialmente associada a resultados clínicos negativos, deve guiar a prática clínica. Os fármacos com efeitos anticolinérgicos estão incluídos nos critérios explícitos de identificação de medicação potencialmente inapropriada em idosos, uma vez que estão associados ao desenvolvimento de outcomes clínicos negativos. Contudo, continuam a ser pouco reconhecidos quanto às suas propriedades anticolinérgicas. Deste modo, e considerando o conceito de carga anticolinérgica – efeito cumulativo da toma de um ou mais fármacos com propriedades anticolinérgicas –, um tipo específico de critérios explícitos foi desenvolvido para utilização em procedimentos de revisão da medicação: as escalas e índices de quantificação da carga anticolinérgica. Assim, tendo em conta que o uso de medicamentos com efeitos anticolinérgicos constitui, muitas vezes, o único fator de risco modificável para prevenir o desenvolvimento de efeitos anticolinérgicos adversos, foi objetivo deste trabalho de investigação analisar de que forma os instrumentos de quantificação da carga anticolinérgica podem ser utilizados na prática clínica como preditores do aparecimento de resultados clínicos negativos nos idosos. Começámos por realizar um estudo de revisão que pretendeu explorar o racional e bases farmacológicas dos instrumentos de quantificação da carga anticolinérgica e avaliar a evidência sobre a sua associação com o desenvolvimento de outcomes negativos. Foi possível perceber que estes apresentam várias lacunas sob o ponto de vista das bases farmacológicas, sendo que muitos se basearam na técnica laboratorial da atividade anticolinérgica do soro, que apresenta limitações importantes, e também em opiniões subjetivas de peritos. Além disso, pôde concluir-se que nenhum instrumento foi considerado como uma opção para utilização universal. Foi também possível verificar que a associação entre as diferentes escalas e índices e o desenvolvimento de outcomes clínicos adversos é muito heterogénea, não sendo possível inferir sobre quais os instrumentos com melhor capacidade preditiva. Posteriormente, foram avaliadas as medidas de efeito da associação entre a carga anticolinérgica, determinada pelas 4 ferramentas que têm maior relevância internacional – Anticholinergic Drug Scale (ADS), Anticholinergic Risk Scale (ARS), Anticholinergic Cognitive Burden Scale (ACB) e Drug Burden Index (DBI) – e outcomes anticolinérgicos adversos periféricos e centrais. Para isso, foi realizado um estudo caso-controlo, em enfermarias de Medicina Interna de um hospital terciário, em indivíduos com mais de 65 anos. Os outcomes periféricos (boca seca e olho seco), centrais (comprometimento cognitivo) e o declínio funcional foram avaliados através de testes/questionários validados. Foi também analisada a associação com quedas. As medidas de efeito foram calculadas através da determinação da AUC das curvas ROC (receiver operating characteristic). Os resultados obtidos demonstraram que as medidas de efeito variaram de “fracas” a “razoáveis”, o que indica que a utilidade das escalas em intervenções que visem reduzir os efeitos anticolinérgicos adversos pode ser limitada. O objetivo seguinte foi explorar até que ponto as medidas de efeito melhoravam se, em vez de se considerar a carga anticolinérgica cumulativa calculada a partir de 14 instrumentos, se considerasse apenas o número de fármacos, de acordo com a lista de fármacos fornecido por cada escala, ou o score do DBI, tendo como ponto de partida as referidas listas de fármacos. Contudo, os resultados obtidos mostraram que a capacidade preditiva foi semelhante, independentemente da abordagem considerada para estimar o perfil de risco dos indivíduos. Estes resultados corroboraram as fragilidades farmacológicas identificadas no trabalho de revisão. Deste modo, o objetivo subsequente foi o de analisar os fármacos incluídos nas diferentes escalas, sob o ponto de vista farmacológico, nomeadamente qual a evidência existente sobre o seu antagonismo em relação aos cinco subtipos de recetores muscarínicos, bem como avaliar o potencial de cada fármaco atravessar a barreira hematoencefálica, para uma melhor identificação do potencial para desenvolver efeitos centrais. Para isso, compilou-se o maior número de escalas até então considerado e avaliou-se cada fármaco individualmente através de uma pesquisa em 4 bases de dados distintas, complementando-se a investigação com uma pesquisa na PubMed. Os resultados mostraram que em menos de metade dos fármacos se reporta afinidade para os diferentes subtipos de recetores muscarínicos. Estes dados levantam dúvidas sobre se os instrumentos existentes são úteis para auxiliar nas decisões clínicas, corroborando os resultados obtidos quanto às medidas de efeito. Foi possível perceber que os instrumentos disponíveis têm um grande potencial para ser aperfeiçoados e que a sua aplicabilidade é limitada. Um contributo importante foi dado para a criação de uma lista universal de fármacos com propriedades anticolinérgicas, que apresenta de forma discriminada as diferentes afinidades para os vários subtipos de recetores muscarínicos. Assim, será possível a obtenção de ferramentas válidas que auxiliem verdadeiramente a prática clínica.
Population aging is a reality that characterizes current societies. Older people are the main consumers of medicines, frequently using a high number of them simultaneously. However, the use of medicines is associated with potential serious iatrogenic harm that becomes particularly relevant in the aged population, where the aging-related physiological changes lead to a larger susceptibility to adverse effects. The difference between appropriate polypharmacy, which is therapeutically beneficial, and inappropriate polypharmacy, potentially associated with negative clinical outcomes, should guide clinical practice. Drugs with anticholinergic effects are included in explicit criteria identifying potentially inappropriate medications in elderly, since they are associated with the development of negative clinical outcomes. However, they remain being little recognized concerning their anticholinergic properties. So, considering the concept of anticholinergic burden – the cumulative effect of using one or more medicines with anticholinergic effects –, a specific type of explicit criteria was developed to be used in medication reviews: anticholinergic burden scales and indexes. Thus, and having in mind that the use of medicines with anticholinergic effects corresponds, very often, to the only modifiable risk factor to prevent the development of adverse anticholinergic effects, the aim of this research was to assess how anticholinergic burden scales and indexes can be used in the clinical practice as predictors of negative clinical outcomes in older people. We started by performing a literature review that intended to examine the rational and pharmacological basis of anticholinergic burden tools and to evaluate the existing evidence about the association of these tools with the occurrence of negative clinical outcomes. With this work, it was possible to recognize that the tools show several gaps concerning the pharmacological basis. Actually, many of them were based on the laboratory assay for serum anticholinergic activity, which have important limitations, and also on subjective experts’ opinions. Additionally, we could conclude that no existing tool was considered as an option for universal use. Moreover, it was possible to substantiate that the association among the different scales and indexes and the development of negative clinical outcomes is very heterogeneous. Existing studies have inconsistent results and so it is not possible to infer which tool has better predictive ability to the different outcomes. Subsequently, the effect size measures of the associations between anticholinergic burden, determined by the 4 most relevant tools – Anticholinergic Drug Scale (ADS), Anticholinergic Risk Scale (ARS), Anticholinergic Cognitive Burden Scale (ACB) and Drug Burden Index (DBI) –, and peripheral and central anticholinergic adverse effects were evaluated. A case-control study was conducted in patients over 65 years admitted to Internal Medicine wards of a tertiary hospital. Peripheral (dry mouth and dry eye) and central (cognitive impairment) adverse outcomes as well as functional impairment were evaluated through validated tests/questionnaires. It was also evaluated the association with falls. The effect size measures were calculated through the determination of the area under the curve (AUC) of receiver operating characteristic (ROC) curves. The obtained results demonstrated that the effect size measures ranged from “fail” to “fair”, which may indicate that the usefulness of scales and indexes in interventions that aim to reduce anticholinergic adverse effects may be limited. The following objective was to explore if the effect size measures improved if, instead of considering the cumulative anticholinergic burden calculated from 14 different scales, we considered only the number of drugs, according to the list of drugs provided by each scale, or the score of the DBI, having as beginning point the mentioned lists of drugs with potential anticholinergic activity. However, the obtained results showed that the predictive ability of the analyzed tools was similar, regardless the approach considered to estimate the risk profile of the included individuals. These results supported the pharmacological weaknesses identified in the revision work. The subsequent objective was to analyze the drugs included in the different scales from a pharmacological point of view, namely the existing evidence about their antagonism concerning the five subtypes of muscarinic receptors. We also aimed to evaluate the potential of each drug to cross the blood brain barrier, for a better identification of the potential to produce central effects. For that, it was compiled the greatest number of scales so far considered simultaneously. Each drug was individually assessed through a research in 4 different databases, supplementing the investigation with a research in PubMed. The results of the research showed that less than a half of the drugs included in the scales present affinity to the different muscarinic receptors subtypes. These results raise concerns about if the existing tools are useful to help the clinical decisions of the healthcare professionals. The information obtained corroborates the results concerning the effect size measures. It was possible to realize that currently available scales and indexes have a great potential to be improved and their usefulness in clinical practice is limited. An important contribution was given for the creation of a universal list of drugs with anticholinergic properties, which presents, in a discriminating way, the different affinities of the drugs to the different muscarinic receptors subtypes. This way, it will be possible to obtain valid tools that truly support the clinical practice.
