Abstract: Objetivo: O propósito deste trabalho foi rever sistematicamente a literatura relacionada com a terapêutica farmacológica da perturbação de hiperatividade e défice de atenção (PHDA) em idade pediátrica e as suas repercussões clínicas, com particular ênfase nas relacionadas com: 1) Qualidade de vida; 2) Psicopatologia global; 3) Desempenho académico e 4) Impacto no desenvolvimento global da criança, de modo a estabelecer clara e inequivocamente a vantagem da prescrição, considerando as repercussões desta em idade pediátrica.Métodos: Foram consultadas a base de dados da PubMed, utilizando como termos MeSH: "attention deficit disorder with hyperactivity", "drug therapy", "treatment outcome" e "child", bem como a Embase e Cochrane Library. Adicionaram-se os filtros: publicados nos últimos 10 anos, Humanos, Inglês, Português, Espanhol. Foram aplicados critérios de elegibilidade.Critérios de elegibilidade: Utilizaram-se estudos que comparavam as consequências em saúde do tratamento em indivíduos com PHDA não tratados com indivíduos com PHDA sob terapêutica farmacológica ou indivíduos com PHDA antes e após início da terapêutica farmacológica.Resultados: De uma inicial pesquisa de 888 artigos, incluíram-se 34 trabalhos nesta revisão.1) Qualidade de vida: Melhoria da qualidade de vida dos indivíduos sob terapêutica farmacológica, a curto-prazo.2) Psicopatologia global: Redução de parte da psicopatologia associada, porém registaram-se em alguns, casos de ideação suicida.3) Desempenho académico: Apesar de se registarem melhorias a nível linguístico, as diferenças são pouco significativas face aos indivíduos que não estão sob terapêutica farmacológica e não conseguem diluir a desvantagem face a indivíduos sem doença.4) Impacto no desenvolvimento global da criança: Apenas a higiene do sono parece beneficiar da farmacoterapia. As variáveis do crescimento (peso, altura e metabolismo ósseo) estão comprometidas. A nível cardiovascular, verificam-se alterações da frequência cardíaca e tensão arterial, bem como perturbações do ritmo, desconhecendo-se, todavia, eventos cardíacos graves.Discussão: O tratamento farmacológico da PHDA parece ser favorável a nível da psicopatologia global, porém verificaram-se em alguns casos, comportamentos suicidas. Em termos de qualidade de vida destes indivíduos, não é a redução dos sintomas proporcionada pela terapêutica que parece melhorar os índices de qualidade de vida e a qualidade de vida relacionada com a saúde evidenciada a curto-prazo. No âmbito do desempenho académico, a vantagem observada a nível linguístico nos indivíduos sob farmacoterapia face aos não medicados, é insuficiente face à disparidade global encontrada relativamente a indivíduos sem doença. No desenvolvimento global da criança e adolescente, somente no domínio da higiene do sono é que a terapêutica farmacológica poderá ser favorável. A farmacoterapia tem um impacto negativo a nível do crescimento avaliado por variáveis como o peso, altura e metabolismo ósseo, bem como a nível cardiovascular por condicionar alterações da frequência cardíaca, tensão arterial e em menor grau, perturbações de ritmo, apesar de não se registarem eventos cardíacos graves. Conclusão: Não foi possível estabelecer inequivocamente que a abordagem terapêutica com recurso a fármacos seja eficiente.
Objectives: The purpose of this study was to review the literature on clinical outcomes of pharmacological therapy of attention deficit and hyperactivity disorder (ADHD) in paediatric ages, specially the ones related to 1) Quality of life; 2) Global psychopathology; 3) Academic achievement; and 4) Global Development of the Child, to stablish if there is a clear and unequivocal vantage of prescribing this treatment, regarding the effects they have in paediatric ages.Methods: For the review we researched in databases of PubMed, Cochrane Library and Embase, using the following MeSH terms: "attention deficit disorder with hyperactivity", "drug therapy", "treatment outcome" e "child", adding the filters: published in the last 10 years, Humans, English, Portuguese, Spanish. Inclusion and exclusion criteria were applied.Selection criteria: Studies comparing the clinical outcomes in individuals with ADHD without comorbidities with no pharmacological treatment with individuals with ADHD without comorbidities pharmacological treated or studies comparing individuals before and after starting pharmacotherapy.Results: Out of 888 initial articles, applied the eligibility criteria we selected 34 articles.1) Quality of life: Improvement on quality of life of individuals under pharmacological therapy in the short term.2) Global Psychology: Reduction of part of the associated psychopathology. Some cases of suicidal ideation were observed.3) Academic achievement: Although there are improvements in the linguistic level, the differences are not very significant in the case of individuals who are not under pharmacological therapy and cannot dilute the disadvantage in relation to individuals without ADHD.4) Global Development of the Child: Only sleep hygiene seems to benefit from pharmacotherapy. Growth variables (weight, height and bone metabolism) are compromised, as well as negative cardiovascular consequences such as changes in heart rate and blood pressure, as well as rhythm disturbances, but serious cardiac events are not known.Discussion: The pharmacological treatment of ADHD seems to be favourable in the global psychopathology, but in some cases, suicidal behaviours have been observed. In terms of the quality of life of these individuals, it is not the reduction of the symptoms provided by the therapy that seems to improve the indexes of quality of life and health-related quality of life evidenced in the short term. In terms of academic performance, the linguistic advantage of individuals under pharmacotherapy compared with unmedicated individuals is insufficient due to the global disparity found in individuals without disease. In the overall development of children and adolescents, only in the field of sleep hygiene can pharmacological therapy be favourable. Pharmacotherapy has a negative impact on growth as measured by variables such as weight, height and bone metabolism, as well as at cardiovascular level by conditioning changes in heart rate, blood pressure and, to a lesser extent, rhythm disturbances, although they do not register cardiac events. Conclusion: It was not possible to establish unequivocally that pharmacological therapy is efficient.
