Abstract: A Síndrome de Hutchinson-Gilford (HGPS) é uma forma de Progeria rara e fatal provocada pela mutação c.1824C>T no exão 11 do gene LMNA. Carateriza-se sobretudo por envelhecimento prematuro cutâneo, ósseo e cardiovascular. Estes indivíduos por norma não completam a maturação sexual, pelo que teoricamente não têm capacidade reprodutiva.O objetivo deste trabalho é apresentar o caso de uma jovem com HGPS que, embora não ultrapasse o estádio M2P1 de Tanner, apresenta menarca espontânea aos 16 anos e ciclos regulares. A história de amenorreia secundária associada às alterações encontradas no estudo hormonal da doente poderá corresponder ao diagnóstico de Síndrome do Ovário Poliquístico (PCOS). O estudo ecográfico revelou aspetos compatíveis com resposta ovárica à FSH e estruturas genitais sem anomalias.Os mecanismos subjacentes à existência de menarca e ciclos regulares nesta e noutras doentes permanecem por determinar tendo em conta o desenvolvimento pubertário e as características da doença. Com base no conhecimento atual também não se consegue estabelecer uma relação de causalidade entre a HGPS e o possível diagnóstico de PCOS .
Hutchinson - Gilford syndrome (HGPS) is a rare and fatal form of Progeria, resulting from the mutation c.1824C>T in exon 11 of the LMNA gene. It is mainly characterized by premature cutaneous, bony and cardiovascular ageing. These individuals usually do not become sexually mature, therefore, in theory are infertile.The aim of this report is to present the case of a young girl with HGPS that, despite classified Tanner stage M2P1, referred having spontaneous menarche at 16 years old and regular menstrual cycles. The history of secondary amenorrhea associated with the hormonal alterations may correspond to the diagnosis of Polycystic Ovary Syndrome (PCOS). The ultrasound revealed aspects compatible with ovary response to FSH and normal genital structures.The mechanisms underlying the existence of menarche and regular menstrual cycles in this and other patients remains unknown, taking into account the pubertal development and characteristics of this disease. According to present knowledge, it is also impossible to establish a causal relationship between HGPS and the possible diagnosis of PCOS .
