Desnutrição no idoso: a problemática da Síndrome de Realimentação

Abstract

In Portugal, there has been an increase in the elderly population. Aging can be defined as a process of progressive decline in biological functions, with an increase in the incidence of chronic diseases. One of the main concerns related to aging is malnutrition, in which the treatment is nutritional therapy. However, if this treatment is performed inappropriately, malnutrition may complicate leading to Refeeding syndrome. This clinical entity is characterized by electrolyte and vitamin changes, and by diverse symptoms, and may even be fatal. Therefore, given the severity of this syndrome and its impact on its prognosis on patient, the main goal of this work is to alert to the problematic of an aggressive refeeding of an elderly malnourished patient. Searches were carried out between Sptember 2018 and Mach 2019. The databases used were Pubmed®, from which most articles were extracted, UpToDate®, Google Scholar® and Scielo/Lilacs®. Articles published in the last 10 years have been favored.Malnutrition is a global reality in which, against all expectations, it also exists in developed countries, namely in the most fragile age groups, such as the elderly. It is associated with an increase in morbidity, mortality, and a decline in quality of life. In order to prevent complications, namely Refeeding Syndrome, it is important to identify patients at risk, to prescribe nutritional therapy in a prudent and appropriate manner, and to correct vitamin and electrolyte deficits. Therefore, it is crucial that the health teams are awake to this problem and are able to recognize this Syndrome, in which can be fatal.

Em Portugal, tem-se verificado um aumento da população idosa. O envelhecimento traduz-se por declínio progressivo das funções biológicas, verificando-se um aumento da incidência de doenças crónicas. Uma das principais preocupações, associadas ao envelhecimento, é a desnutrição, a qual tem como tratamento a terapêutica nutricional. Contudo, se este tratamento for realizado de forma desajustada, a desnutrição poderá complicar-se levando ao aparecimento da Síndrome de Realimentação. Esta entidade clínica é caracterizada por alterações eletrolíticas e vitamínicas, e sintomatologia diversa, podendo, inclusive, levar à morte. Assim, dada a gravidade desta síndrome e o impacto da mesma no prognóstico dos doentes, o objetivo principal deste trabalho consiste em alertar para a problemática associada à realimentação inadequada de um idoso desnutrido.A pesquisa bibliográfica foi realizada, no período entre Setembro de 2018 e Março de 2019. Para tal, foram utlizadas as bases de dados, Pubmed®, de onde se extraíram a maioria dos artigos, Embase®, UpToDate®, Google Scholar® e Scielo/Lilacs®. Privilegiaram-se artigos publicados nos últimos 10 anos.A desnutrição é prevalente entre a população geriátrica e, para se prevenirem complicações como a Síndrome da Realimentação, é importante identificar os doentes em risco, instituir terapia nutricional de forma prudente e adequada e corrigir os défices vitamínicos e eletrolíticos. Para tal, é fundamental que as equipas de saúde estejam despertas para esta problemática e saibam reconhecer esta Síndrome que, por sua vez, poderá ser fatal.
