Resumo: Methylphenidate (MPH) is the first-line medication prescribed for attention deficit hyperactivity disorder (ADHD) treatment. The increase in both ADHD diagnosis and MPH misuse has been raising alarm among clinicians and population in general. In fact, MPH has become very popular among healthy young adults that seek out cognitive enhancement and improvement of their physical performance. Currently knowledge is scarce about both the effects of MPH on a non-diseased brain and the long-term consequences of its consumption. Therefore, it is highly important to unravel how MPH use affects the brain of healthy individuals.Microglia are the brain resident immune cells responsible for central homeostasis and defense against invading pathogens. These cells are also constituents of the neurovascular unit (NVU) and present a bidirectional communication with other brain cells. Upon activation, microglia exhibit an inflammatory phenotype that has a positive role in brain protection and homeostasis, which contributes to debris clearance and tissue regeneration. However, in contrast to this first acute response, chronic inflammation will lead to deleterious consequences, including neurodegeneration. Despite the importance of microglial cells in normal brain function and disease, not much is known about the impact of MPH on these cells. Therefore, the present work intended to assess direct and brain endothelial cells-mediated effects of MPH on microglial cells.The results show that direct MPH treatment of microglial cells (N9 and HMC3 cell lines) did not induce alterations in TNF release, reactive oxygen species and nitric oxide production, and phagocytic activity. However, MPH was able to inhibit microglial cells migration. Furthermore, the role of MPH in the crosstalk between endothelial cells and microglia was investigated. It was also revealed that conditioned medium obtained from brain endothelial cells exposed to MPH led to a reduction in TNF released and phagocytic efficiency by microglia. Overall, this work showed that direct MPH exposure did not originate an inflammatory response by microglial cells. Nevertheless, MPH reduced cell motility. Curiously, it was observed an impairment of phagocytic efficiency and release of TNF by microglial cells when exposed to endothelial cells conditioned medium obtained from cells previously incubated with MPH. In sum, it can be concluded that microglia responses mainly occurred through an indirect way, via endothelial cells. These observations highlight the importance of investigating the crosstalk between different neural cells, which is particularly important at the NVU since endothelial cells are the first to contact with peripheral factors. This study was the first to show this crosstalk under MPH exposure and further studies will be crucial to further characterize such crosstalk and consequences.
O metilfenidato (MTF) é o psicoestimulante de primeira linha prescrito para o tratamento da perturbação de hiperatividade/défice de atenção (PHDA). Ultimamente, o aumento do número de diagnósticos de PHDA bem como do consumo não terapêutico de MTF têm originado uma crescente preocupação entre os clínicos e a população em geral. De facto, o consumo de MTF tornou-se bastante popular entre jovens adultos que procuram uma melhoria das suas capacidades cognitivas e do desempenho físico. Atualmente, pouco se sabe sobre os efeitos do MTF no cérebro numa condição fisiológica e quais são as consequências do seu consumo a longo prazo. Como tal, é muito importante estudar de que forma é que o consumo de MTF afeta o cérebro de indivíduos saudáveis.As células da microglia são consideradas o sistema imunitário do cérebro, sendo responsáveis por manter a homeostasia e conferir proteção contra agentes patogénicos. Estas células fazem também parte da unidade neurovascular e, por isso, interagem bidirecionalmente com outras células. Após ativação, estas células exibem um fenótipo inflamatório importante na proteção cerebral e na homeostasia, contribuindo para a limpeza de resíduos celulares e regeneração do tecido. No entanto, em contraste com esta primeira resposta aguda, a inflamação crónica tem consequências nefastas, como é exemplo a neurodegeneração. Apesar da sua importância tanto no normal funcionamento do cérebro como numa situação de doença, pouco se sabe sobre o impacto do MTF na microglia. Neste sentido, o presente trabalho teve como principal objetivo avaliar os efeitos do MTF na microglia, tanto de forma direta, como de forma indireta, mediados por células endoteliais cerebrais.Os resultados mostram que a exposição direta da microglia (linhas celulares N9 e HMC3) a MTF não induziu alterações na libertação de TNF, produção de espécies reativas de oxigénio e óxido nítrico, bem como da atividade fagocítica. Porém, o MTF teve um efeito inibitório na migração destas células. Para além disso, foi avaliado o papel do MTF na comunicação entre células endoteliais e microglia. Nesse sentido, foi possível verificar que as células da microglia, quando expostas a meio condicionado obtido de células endoteliais cerebrais previamente incubadas com MTF, apresentaram uma redução da libertação de TNF e da eficiência fagocítica.Em suma, o presente estudo demonstrou que a exposição direta da microglia a MTF não induziu uma resposta inflamatória por parte destas células. Apesar disso, o MTF foi capaz de reduzir a migração celular. Curiosamente, observou-se uma diminuição da eficiência de fagocitose e da libertação de TNF por parte da microglia quando incubadas com meios condicionados de células endoteliais expostas a MTF. Concluindo, a resposta da microglia ao MTF foi essencialmente mediada por células endoteliais. As evidências aqui apresentadas exaltam a importância do estudo da comunicação entre diferentes tipos de células do cérebro. Este trabalho foi o primeiro a identificar de que forma o MTF modula esta comunicação e futuros estudos serão essenciais para caracterizar pormenorizadamente as consequências das alterações desta comunicação entre células endoteliais e microglia.
