Abstract: Introdução: A doença litiásica é uma doença comum um pouco por todo mundo e estima-se que afete mais de 10% da população mundial. Os cálculos urinários podem ocorrer em qualquer local do trato urinário e são frequentemente, assintomáticos quando localizados na pélvis renal e/ou na bexiga. No entanto, são causa frequente de obstrução ureteral sintomática parcial ou completa. Atualmente, o tratamento standard para a remoção/fragmentação de cálculos uretéricos é a ureterorrenoscopia – UR. Contudo, nem todas as UR resultam na identificação de cálculo passível de ser removido, por este já não se encontrar no sistema coletor. Esta situação é referenciada na literatura como ureteroscopia branca – URB, do inglês negative ureteroscopy. O objetivo deste trabalho é identificar a prevalência de URB no CHUC, bem como possíveis fatores preditivos desta situação, de modo a definir medidas que possam contribuir para a expulsão espontânea do cálculo. Pretende-se contribuir para uma redução de custos inerentes à intervenção cirúrgica, otimizar os cuidados de saúde e reduzir o impacto de tratamentos desnecessários.Material e métodos: Realizámos um estudo retrospetivo tipo coorte, no qual analisámos informações clínicas de 262 doentes que foram sujeitos a ureteroscopia por doença litiásica no CHUC entre 1 de julho de 2016 e 30 de junho de 2018.Resultados: Foram incluídos 262 doentes no estudo, 50,8% do sexo feminino e 49,2% do sexo masculino, com média de idades de 56,89 anos. A ureteroscopia foi considerada branca em 29,8% dos casos. Das variáveis estudadas, as que se relacionaram com URB foram o facto de ser um cálculo primário (p=0,001), ser do sexo feminino (p=0,023), a relação de proporcionalidade indireta entre o tamanho do cálculo e a probabilidade de ser branca (p=0,010) e a radiotransparência do cálculo (p=0,035).Conclusão: A ureteroscopia foi considerada branca numa taxa muito elevada. Indivíduos do sexo feminino, cálculos primários, de tamanho inferior a 1 cm e radiotransparentes são fatores que contribuem para a maior ocorrência de URB. A idade do doente e a localização do cálculo, bem como o tempo decorrido entre o episódio agudo/derivação e a data da UR, não se revelaram preditivos para URB.
Introduction: Lithiasis is a common disease worldwide, it affects an estimated 10% of the world population. Urinary calculi can occur at any site of the urinary tract and are often asymptomatic when located in the renal pelvis and/or bladder. However, they are a frequent cause of partial or complete symptomatic ureteral obstruction. Currently, the standard treatment for removal/fragmentation of ureteric stones is ureteroscopy. However, not all ureteroscopies result in the identification of a treatable stone, since it may no longer be found in the collecting system. This situation is referred to in the literature as negative ureteroscopy. The objective of this study is to identify the prevalence of negative ureteroscopy in CHUC, as well as possible predictive factors of this situation, in order to define measures that may contribute to the spontaneous expulsion of the stone. It is intended to contribute to a reduction of costs inherent to surgical intervention, to optimize health care and to reduce the impact of unnecessary treatments.Patient and methods: We performed a retrospective cohort study in which we analysed clinical information from 262 patients who underwent ureteroscopy due to urolithiasis at CHUC from July 1, 2016 until June 30, 2018.Results: A total of 262 patients were included in the study, 50.8% female and 49.2% male, with a mean age of 56.89 years. Ureteroscopy was considered negative in 29.8% of cases. Of the variables studied, those that were related to a negative UR included that it was a primary stone (p=0.001), being female (p=0.023), the ratio of indirect proportionality between the size of the stone and the probability of being negative (p=0.010) and radiolucency of the stone (p=0.035).Conclusions: Ureteroscopy was considered negative at a very high rate. Female individuals, primary calculi, less than 1 cm in size and radiolucent are factors that contribute to the higher occurrence of a negative ureteroscopy. The age of the patient and the location of the calculi, as well as the time elapsed between the acute episode/derivation and the date of UR, were not predictive of a negative ureteroscopy.
