Resumo: A doença de Parkinson é a segunda doença neurodegenerativa mais comum, afetando cerca de 1% da população mundial com idade superior a 65 anos. Caracteriza-se principalmente pela degenerescência dos neurónios dopaminérgicos na substância negra, envolvendo também diferentes regiões cerebrais. Avanços nos estudos de neuroimagem molecular, estrutural e funcional têm aumentado o conhecimento acerca das diversas síndromes parkinsónicas, salientando-se o uso da imagem de ressonância magnética estrutural na segmentação do sistema nervoso e na deteção de possíveis alterações de volume cerebral. O estudo de alterações volumétricas cerebrais permite inferir sobre a relação entre as possíveis alterações cerebrais características das diferentes síndromes parkinsónicas e os sintomas característicos dessas síndromes, apresentando-se assim como um complemento ao diagnóstico diferencial da doença de Parkinson, cujo diagnóstico é realizado com base em critérios clínicos e neuropatológicos observados nos doentes.Neste contexto surgiu o tema deste trabalho cujo principal objetivo foi estudar as potencialidades do uso de imagens de ressonância magnética estrutural no diagnóstico diferencial das síndromes parkinsónicas. Para tal, foi usado um dataset, disponibilizado pelo ICNAS, constituído por 62 imagens de ressonância magnética estrutural (correspondentes a 14 controlos, 26 doentes com doença de Parkinson (DP), 8 doentes com atrofia de sistemas múltiplos (AMS), 7 doentes com degenerescência corticobasal (DCB) e 7 doentes com demência com corpos de Lewy (DCL)) e aplicada uma análise estatística paramétrica recorrendo ao software Matlab R2015b e à toolbox SPM12. Foi ainda avaliada a capacidade de classificar individualmente as diferentes síndromes parkinsónicas recorrendo aos dados volumétricos obtidos na análise estatística paramétrica. Para tal, foram criados classificadores no sotfware Matlab R2015b, utilizando o método SVM e a sua biblioteca LIBSVM.A análise volumétrica realizada permitiu identificar regiões cerebrais com diferenças estatisticamente significativas de volumes médios de substância cinzenta e branca nas diferentes síndromes parkinsónicas e associar as alterações observadas aos sintomas clínicos característicos das síndromes em estudo. As comparações em estudo foram incluídas nos grupos síndrome versus controlo, síndromes versus controlo, síndromes versus síndromes e ainda, síndrome versus síndrome. A comparação volumétrica DP versus controlo, permitiu observar uma diminuição de substância cinzenta no lobo parietal e no lado esquerdo do lobo temporal nos doentes com DP. Por outro lado, a comparação entre doentes com AMS e indivíduos saudáveis indicou uma atrofia do lado esquerdo do lobo temporal dos doentes com AMS. O estudo da substância cinzenta entre doentes com DCB e indivíduos saudáveis demonstrou, por sua vez, atrofias do lado esquerdo do lobo frontal e do lado direito dos lobos occipital e temporal nos cérebros dos doentes com DCB, que poderão explicar alguns dos sintomas associados à DCB. A comparação entre DCL e controlo indicou uma diminuição bilateral de volume médio de substância cinzenta nos lobos frontal, temporal e parietal e no lado direito do lobo occipital dos doentes com DCL.As classificações efetuadas apresentaram, na sua maioria, valor clínico e estatístico podendo representar uma mais-valia no diagnóstico clínico individual das síndromes parkinsónicas. As classificações binárias indicaram valores de sensibilidade, especificidade e precisão balanceada contidos em intervalos de 71%-88%, 71%-100% e 72%-93%, respetivamente. Observou-se uma exceção no caso da classificação entre os grupos de doentes com DCL e doentes com DCB, na qual a classificação não revelou ser significativa. A classificação multiclasse realizada com os 4 grupos de doentes revelou uma precisão significativa de 46%, acima do nível do acaso.
Parkinson's disease is the second most common neurodegenerative disease, affecting about 1% of the world's population over 65 years of age. It is mainly characterized by the degeneration of dopaminergic neurons in the substantia nigra, also involving different brain regions. Advances in molecular, structural and functional neuroimaging studies have increased the knowledge about the neurobiological complexity of the various parkinsonian syndromes, emphasizing the use of structural magnetic resonance imaging in the segmentation of the nervous system and in the detection of possible changes in mean brain volume. The study of cerebral volumetric alterations allows to infer about the relation between the possible cerebral alterations characteristic of the different parkinsonian syndromes and the characteristic symptoms of these syndromes, presenting as a complement to the differential diagnosis of Parkinson's disease, which diagnosis is based on clinical criteria and neuropathological conditions observed in patients.In this context the theme of this work appeared, which main objective was to study the potentialities of the use of structural magnetic resonance imaging in the differential diagnosis of the parkinsonian syndromes. For this, a dataset from ICNAS was used, consisting of 62 structural magnetic resonance images (corresponding to 14 control subjects, 26 patients with Parkinson's disease (PD), 8 patients with multiple system atrophy (MSA), 7 patients with corticobasal degeneration (CBD) and 7 patients with dementia with Lewy bodies (DLB)) and was applied a parametric statistical analysis using the software Matlab R2015b and toolbox SPM12.The ability to classify the different parkinsonian syndromes individually using the volumetric data obtained in the parametric statistical analysis was also evaluated. For this, classifiers were created in the Matlab R2015b software, using the SVM method and its LIBSVM library.The volumetric analysis allowed to identify cerebral regions with statistically significant differences of gray and white matter volumes in the different parkinsonian syndromes and to associate the observed changes with the clinical symptoms characteristic of the syndromes under study. The comparisons under study were included in the groups syndrome versus control, syndromes versus control, syndromes versus syndromes and syndrome versus syndrome. Volumetric comparison of DP versus control showed a decrease in gray matter in the parietal lobe and in the left side of the temporal lobe in patients with PD. On the other hand, the comparison between patients with MSA and healthy individuals showed left atrophy of the temporal lobe of MSA patients. The study of gray matter between CBD patients and healthy individuals has shown atrophies on the left side of the frontal lobe and on the right side of the occipital and temporal lobes in the brains of patients with CBD who may explain some of the symptoms associated with CBD. The comparison between LBD and control indicated a bilateral decrease in mean gray matter volume in the frontal, temporal and parietal lobes and in the right side of the occipital lobe of the patients with LBD.The classifications performed showed, in the majority, clinical and statistical value and could represent an added value in the individual clinical diagnosis of the parkinsonian syndromes. Binary classifications indicated values of sensitivity, specificity and balanced precision contained in intervals of 71%-88%, 71%-100% and 72%-93%, respectively. An exception was noted in the case of classification between the groups of patients with LBD and patients with CBD, in which the classification did not prove to be significant. The multiclass classification performed with the 4 patient groups showed a significant accuracy of 46%, beyond chance levels.
