Abstract: Several studies emphasize the benefits of colorectal cancer screening in its mortality, and the low adherence to the screening leads to the search of new approaches to this matter. In this study, we examined the influence of the populations’ health beliefs and sociodemographic variables in the screening adherence, as well as the correlation of the empathy between the physician and the patient with the screening adherence and the health beliefs itself. A total of 122 women and men between the ages of 50 and 74 years old and with the criteria for CRC screening according to DGS, were evaluated with the Health Belief Model and the Jefferson Scale of Patient Perception of Physician Empathy. We verified a low adherence behavior (40,2%), on which only 46,7% went through a colonoscopy and 25,4% had done a fecal occult blood testing. Overall, only 48,7% of the patients were recommended by the physician to perform the screening. It was shown, with significance, that the adherent sample had an age mean higher than the non-adherent (p=0,005) and that the physician recommendation and the Barrier perception are significant predictors of the screening adherence (p=0,001). It was demonstrated a direct relation between Benefits and Empathy (r=0,031, p=0,000), and an inverse relation between Barriers and Empathy (r=-0,104, p=0,290). This could be used as a form of intervention on the patients’ health beliefs, contributing indirectly to the increase of adherence to the screening. It was concluded that the patients’ opinion on the psychological and physical consequences of the action, in this case, the performance of the exams, creates a barrier to the screening. With that being said, it is useful to understand the main worries of the patients regarding the exams and try to clarify those fears in the appointment. Other important tool in the increase of the screening adherence, found in this study and in a major part of the literature, is the physician recommendation, which is still very absent in the family doctor appointments.
Vários estudos enfatizam o benefício do rastreio do CCR na sua mortalidade e a baixa adesão populacional leva à procura de novas abordagens para esta questão. Com este estudo pretendemos verificar como as crenças de saúde da população têm efeito na adesão ao rastreio; como as características sociodemográficas da população-alvo estão relacionadas com a adesão ao rastreio e, por fim, explorar a relação entre a empatia do doente com o seu médico de família e a sua adesão ao rastreio.Neste estudo, 122 utentes de centros de saúde com idades compreendidas entre os 50 e 74 anos com os critérios definidos pela DGS para a realização de rastreio do CCR, foram avaliados com a Escala de Crenças de Saúde e o questionário Jefferson Scale of Patient Perception of Physician Empathy.Verificou-se um comportamento de baixa adesão ao rastreio (40,2%) em que apenas 46,7% realizou colonoscopia e 25,4% realizou PSOF. No geral, somente 48,4% dos indivíduos foram aconselhados pelo seu médico de família a realizar o rastreio. Foi demonstrado, com significância, que a população Aderente tinha uma idade média mais alta que a Não aderente (t(-2,87), p=0,005) e que a recomendação médica e a perceção de Obstáculos são preditores significativos para a adesão ao rastreio (p=0,001). Foi demostrada uma relação direta entre os Benefícios e a Empatia (r=0,031, p=0,000), e uma relação inversa entre os Obstáculos e a Empatia (r=-0,104, p=0,290) sendo que esta relação poderá servir como forma de intervenção nas crenças de saúde dos utentes, contribuindo assim de forma indireta para o aumento de adesão no rastreio. Concluiu-se então que a opinião do utente sobre as consequências psicológicas e tangíveis da ação, neste caso a realização do exame, cria uma barreira à adesão do rastreio, sendo útil perceber quais as principais preocupações dos utentes relativamente ao exame e trabalhar no sentido de desmistificar e esclarecer os medos dos utentes nas consultas. Outra ferramenta importante no aumento da adesão ao rastreio, encontrada neste estudo e em grande parte da literatura, é a recomendação médica, que ainda não estará presente de forma rotineira na consulta do Médico de família.
