Título: Obesidade nos cuidados de saúde primários : prevenção

Resumo: A epidemia da obesidade representa actualmente um dos mais graves desafios para a saúde pública, tendo a prevalência da obesidade aumentado para o triplo nos últimos vinte anos, sendo que metade da população adulta e uma criança em cada cinco, sofrem de excesso de peso e destes um terço são mesmo obesos (Região Europeia da Organização Mundial de Saúde). O peso excessivo e a obesidade são factores de risco e potenciadores do agravamento de várias doenças diminuindo a esperança média e a qualidade de vida. As mudanças sociais, económicas, culturais e físicas do meio ambiente contribuíram para este aumento exponencial de prevalência. A redução drástica na prática de exercício físico e a modificação dos padrões alimentares (consumo elevado de alimentos com alto teor de gordura saturada, sal e açúcares simples e a ingestão insuficiente de fruta e vegetais), provocaram um desequilíbrio energético na população. No sentido de ser travada esta epidemia tem de ser encontrado um equilíbrio entre a responsabilidade do indivíduo, da sociedade e dos governos, e têm de ser implementadas medidas preventivas de promoção de estilos de vida saudáveis, como uma alimentação equilibrada e a prática regular de exercício físico. Apesar de em alguns países existirem orientações e “guidelines” nesse sentido há várias décadas, pouco é feito e o pouco que se faz não tem tido repercussões na diminuição da prevalência da doença. Em Portugal as actuações ao nível da prevenção são de um modo geral pontuais e isoladas, não havendo uma estratégia concreta neste âmbito, existindo apenas algumas orientações gerais da Direcção Geral de Saúde. Os Cuidados de Saúde Primários devem assumir um papel impulsionador nas estratégias de prevenção pela sua proximidade às populações e capacidade de intervir na comunidade, que está provado ser o nível de actuação com melhor “outcome” em termos de adopção de estilos de vida saudáveis
The obesity’s epidemic represents at the moment one of the most serious challenges to public health, having the prevalence of obesity increased for the triple in the last twenty years, knowing that half of the adult population and one child in each five, suffer from weight excess and a third of those, are obese (World Health Organization’s European Region). Excessive weight and obesity are risk factors and enhancers for the aggravation of several diseases, reducing the espectancy to live and the quality of life. The social, economical, cultural and physical changes of the environment contributed for this exponential increase of prevalence. The drastic reduction in the practice of physical exercise and the alteration of food standards (elevated consumption of foods with high contents of saturated fat, salt and simple sugars and the insufficient ingestion of fruit and vegetables), caused an energetic imbalance in the population. In the sense of stopping this epidemic it must be found a balance between the individuals, the society and the government’s responsability, and preventive measures promoting healthy life styles must be implemented, such as a balanced nourishment and the practice of regular physical exercise. Although in some countries exists policies and guidelines for several decades, little is done and the little that is truly done doesn’t have repercussions in the reduction of prevalence of the disease. In Portugal the actions at the level of prevention are , in generally, punctual and isolated, not subsisting a concrete strategy in this ambit, existing only general directions of the General Direction of Health. The primary health care sistem must assume a stimulating role in the strategies of prevention for its proximity to the populations and capacity of intervenence in the community, which is proved to be the level of acting with better outcome in terms of adopting healthy life styles.
