nomeadamente as induzidas por antibióticos (ABs), e têm um grande impacto em orientações
terapêuticas posteriores. O seu correto diagnóstico, nomeadamente recorrendo a testes
epicutâneos, permite: identificar os fármacos responsáveis pelas RAMcs, contribuindo assim
para a prevenção de futuras reações pelo mesmo fármaco, por vezes de maior gravidade, e
para limitar o leque de fármacos que o doente ou o seu médico têm receio de utilizar.
Objetivos: Avaliar a reprodutibilidade dos testes epicutâneos positivos a ABs e sua
importância na identificação do fármaco causal em RAMc retardadas clinicamente imputáveis
aos ABs, nos diferentes grupos de ABs e de padrões de reação cutânea em diferentes períodos
após a resolução da RAMc.
Métodos: A informação extraída para este estudo foi obtida através da recolha e avaliação das
fichas alergológicas e processos clínicos dos doentes do Serviço de Dermatologia dos CHUC,
que nos últimos 15 anos tiveram testes epicutâneos positivos a ABs e que repetiram os testes
pelo menos 12 meses mais tarde. A reprodutibilidade dos testes foi avaliada em função das
características demográficas dos doentes, do padrão das RAMc, da classe de AB com testes
positivos, do intervalo de tempo entre a realização dos testes e da intensidade da reação.
Resultados: Avaliou-se um grupo de 20 doentes (12 do sexo feminino e 8 do sexo masculino),
com idades compreendidas entre os 17 e os 72 anos, com antecedentes de RAMc (90% com
exantema maculopapular) clinicamente imputável a um AB e testes epicutâneos iniciais

positivos. O intervalo de tempo entre os testes para cada doente variou entre 1,76 e 14,7 anos.
O teste epicutâneo com o AB imputável foi reprodutível em 15/20 (75%) dos doentes. Das 35
reações observadas nos primeiros testes, 28 (80%) mantiveram-se positivas, sobretudo as
relacionadas com betalactâmicos. A reprodutibilidade foi independente do sexo, idade,
intervalo de tempo entre os testes e da intensidade do teste inicial (p>0,05), tendo contudo
sido observada uma tendência para a redução da intensidade dos testes (p<0,05) ao longo do
tempo.
Discussão: Neste estudo verificámos uma elevada reprodutibilidade dos testes epicutâneos,
mesmo após longos intervalos de tempo entre os testes, sobretudo para os betalactâmicos e
clindamicina, que foram testados em maior número de doentes. A reprodutibilidade dos testes
positivos reforça a importância destes testes epicutâneos no diagnóstico de RAMc retardadas
a ABs e a possibilidade da sua utilização como diagnóstico retrospectivo. Background: Delayed cutaneous adverse drug reactions (CADRs) are common, especially those induced by antibiotics (ABs) and they have a major impact on subsequent treatment. The correct diagnosis, namely using patch testing, often enables the precise identification of the responsible drug and, therefore, prevents future reactions from the same drug and limits the number of drugs the patient has to avoid. Objectives: Evaluate the reproducibility of patch tests in identifying the causative drug in delayed CADRs clinically attributable to ABs, in different groups of ABs and different cutaneous reaction patterns performed in different periods after the resolution of CADRs. Methods: The information for this study was collected through evaluation of medical records of patients of the CHUC Dermatology Service, who had positive patch tests to ABs in the last 15 years and repeated the tests at least 12 months thereafter. The reproducibility of the test was evaluated according to the demographic characteristics of the patients, the pattern of CADRs, the AB class with positive tests, the time period between the tests and the intensity of the reaction. Results: We evaluated 20 patients (12 females and 8 males), aged between 17 and 72 years, with a history of CADRs (90% with maculopapular exanthema) and an initial positive patch test with the imputable antibiotic. The time period between tests for each patient ranged between 1.76 and 14.7 years. Patch tests were reproducible in 15/20 (75%) patients. Among the 35 patch test reactions observed in the first test in these 20 patients, 28 (80%) remained positive, particularly those related to beta-lactams (87,5%) and clindamycin (71,4%). The reproducibility was independent of sex, age, time period between the tests and the intensity of the initial test (p>0.05). However, a tendency to reduce the intensity of tests (p<0.05) was observed over time. 5 Discussion: In this study we found a high reproducibility of patch tests, even after long periods of time between tests, especially for beta-lactams and clindamycin, which have been tested in a greater number of patients.
