Abstract: A mandíbula é o único osso móvel da face e participa de funções básicas, como mastigação, fonação e deglutição, além de participar da manutenção da oclusão dentária. A área da reconstrução mandibular tem evoluído consideravelmente nos últimos 50 anos, no entanto, continua a ser um grande desafio para o cirurgião maxilo-facial, não só pela conformação tridimensional deste complexo, composto por osso e partes moles, como pelos potenciais défices funcionais induzidos, tais como dificuldades na alimentação, comunicação, respiração, incontinência oral, além de resultado estético pobre. A literatura relata diversos métodos de reconstrução, quer por meio de enxerto ósseo convencional ou ósseo vascularizado, quer recorrendo a materiais aloplásticos. Atualmente, o “Gold-standard” da reconstrução de defeitos mandibulares são os retalhos microcirúrgicos com enxertos ósseos vascularizados, sendo os mais utilizados os de fíbula. Com o objetivo de colmatar as limitações dos “Gold-standard” foram surgindo nos últimos anos novas técnicas: a distração osteogénica com disco de transporte; endopróteses modulares e procedimentos na área da engenharia de tecidos. Este trabalho tem como objetivo fazer uma revisão da literatura dos últimos 10 anos, tendo em vista a sumarização dos conhecimentos obtidos até agora, principalmente no que diz respeito às técnicas mais recentes
The mandible is the only mobile bone of the face and participates in basic functions such as mastication, speech and swallowing, as well as participate in the maintenance of dental occlusion. The field of mandibular reconstruction has evolved over the past 50 years, however, it remains a major challenge for the maxillofacial surgeon, not only for the tridimensional conformity of this complex, composed of bone and soft tissue, but also because of the potential induced functional deficits such as difficulty feeding, talking and breathing, oral incontinence, and poor aesthetic result. The literature reports several reconstruction methods, either by conventional or vascularized bone graft or by using alloplastic materials. Currently, the "Gold-standards" of mandibular reconstruction are the flaps with vascularized bone grafts, the most used is the fibular. Aiming to overcome the limitations of the "Gold-standards", new techniques have emerged in the past years: transport disc osteogenic distraction ; modular endoprosthesis and procedures in the field of tissue engineering. This paper aims to review the literature of the last 10 years, in order to summarize the knowledge obtained so far, especially in regard to the most contemporary techniques
