Title: Efeito da Membrana Amniótica no Carcinoma Hepatocelular

Abstract: O carcinoma hepatocelular possui uma elevada incidência e mortalidade, em parte devido ao estadio avançado em que normalmente é diagnosticado e à sua elevada resistência às terapias convencionais, como a radioterapia ou a quimioterapia. Por representar atualmente um problema de saúde pública, é essencial investir em novas terapêuticas para este tipo de cancro. A membrana amniótica humana (hAM) tem sido referenciada em várias publicações como uma potencial opção na terapêutica oncológica devido às suas propriedades anti-angiogénica, pro-apoptótica e imunoregulatória. De facto, vários derivados da hAM, como as suas células ou o seu meio condicionado, foram já utilizados em estudos in vitro e in vivo na terapia de vários tipos de cancro. Até agora, não existem dados sobre o efeito anticancerígeno de extratos proteicos de hAM (hAMPE, do inglês, human amniotic membrane protein extracts). Desta forma, este trabalho experimental teve como principal objetivo estudar o efeito do hAMPE em três linhas celulares humanas de carcinoma hepatocelular: HuH7, HepG2 e Hep3B2.1-7. Foi possível verificar através da técnica de eletroforese 2D que o hAMPE é constituído por um elevado número de proteínas, dentro das quais se encontram as proteínas responsáveis pelos efeitos anti-cancerígenos observados durante o trabalho experimental. O hAMPE induziu uma diminuição da atividade metabólica, do conteúdo proteico total e do conteúdo de ADN total em todas as linhas celulares estudadas. Este tratamento revelou também ser capaz de intervir na regulação do ciclo celular das linhas celulares em estudo. Também se verificouuma indução de morte celular nas três linhas celulares de carcinoma hepatocelular, acompanhada por uma diminuição da expressão das proteínas P53 e β-catenina. A linha celular HepG2 sofreu elevados danos no DNA após tratamento, ao contrário das outras duas linhas celulares, cujo DNA não foi danificado. A terapia combinada utilizando hAMPE e os fármacos 5-Fluorouracil, Doxorrubicina, Cisplatina e Sorafenib, revelou ser uma estratégia promissora por potenciar o efeito anti-cancerígeno dos fármacos já utilizados na clínica. Os resultados in vivo, apesar de preliminares, corroboraram os resultados observados in vitro, sendo que após tratamento com hAMPE se observou uma diminuição da taxa de crescimento tumoral relativamente à condição controlo. Este trabalho revela o potencial anticancerígeno do hAMPE no carcinoma hepatocelular. Estudos relativos à identificação das proteínas constituintes do hAMPE, o aprofundamento dos mecanismos responsáveis pela morte celular e também a realização de um modelo animal ortotópico e posterior terapia constituem algumas das perspetivas futuras deste trabalho.
Hepatocellular carcinoma has a high incidence and mortality, in part due to the late stage at which is diagnosed and its high resistance to conventional therapies, such as radiotherapy or chemotherapy. Actually, it represents a public health problem and, for this reason, it is essential to invest in new therapies for this cancer. The human amniotic membrane (hAM) has been referenced in several publications as a potential therapeutic option in cancer due to its anti-angiogenic properties, pro-apoptotic and immunoregulatory properties. In fact, several derivatives of hAM, as their cells or their conditioned medium, were already used in in vitro and in vivo in several types of cancer therapy. So far, there are no data about the anticancer effect of hAM protein extracts (hAMPE). Thus, this experimental work aimed to study the effect of hAMPE in three human hepatocellular carcinoma cell lines: Huh7, HepG2 and Hep3B2.1-7. It was verified by 2D electrophoresis that hAMPE consists in a large number of proteins, within are the proteins which are responsible for the anticancer effects observed in this experimental work. hAMPE induced a decrease in metabolic activity, total protein content and total DNA content in all cell lines of hepatocellular carcinoma under study. Treatment with hAMPE interfered with cell cycle regulation of all cell lines. There was also an induction of cell death in all cell lines of hepatocellular carcinoma, accompanied with a decreased expression of P53 and β-catenin. The cell line HepG2 suffered high DNA damage after treatment, unlike the other two cell lines, whose DNA was not damaged. The combined therapy ofhAMPE with drug 5-Fluorouracil, Doxorubicin, Cisplatin and Sorafenib therapy has proved to be a promising strategy for enhancing the effect of anticancer drugs. The in vivo results, although preliminary, corroborate the results observed in vitro taking into account that following treatment with hAMPE, a decrease in the rate of tumor growth was observed relatively to control condition. This work reveals the anticancer potential of hAMPE in hepatocellular carcinoma. Studies concerning the identification of proteins in hAMPE, the study of the mechanisms responsible for cell death and also the realization of an orthotopic model and subsequent therapy are some of the future prospects of this work.
