Desenho: Ensaio prospectivo, aleatorizado e controlado numa amostra de 37 doentes com
artrite reumatóide, distribuídos por suplementação com ácidos gordos ómega-3 (n=11), dieta
mediterrânica (n=8) e grupo controlo (n=15), durante 6 meses. Foi avaliado o número de
articulações dolorosas e tumefactas, o estado global de saúde (avaliado pela escala visual
analógica), a velocidade de sedimentação, a proteína C reactiva e foi calculado o índice de
actividade de doença DAS-28 (com velocidade de sedimentação), antes e após a intervenção.
Utilizaram-se os testes de Wilcoxon e Kruskal-Wallis para análise estatística dos resultados
obtidos.
Resultados: Verificou-se uma diminuição significativa da velocidade de sedimentação após 6
meses de suplementação com ácidos gordos ómega-3. Os restantes parâmetros avaliados não
revelaram diferenças significativas ao final dos 6 meses, independentemente do grupo de
estudo e da terapêutica em curso para a artrite reumatóide. Contudo, verificou-se uma
tendência para a redução do DAS-28 e do estado global de saúde avaliado pela escala visual
analógica.
Conclusão: A suplementação com ácidos gordos ómega-3 provocou uma diminuição da
velocidade de sedimentação ao final de 6 meses. A dieta mediterrânica não demonstrou
alteração nos parâmetros avaliados. Observou-se uma tendência para a redução do DAS-28
nos doentes suplementados com ómega-3, levantando-se a necessidade de continuar a estudar
este tipo de suplemento como parte da terapêutica da artrite reumatóide, idealmente com
amostras de maiores dimensões. Aim: To evaluate the effect of omega-3 fatty acids supplements and mediterranean diet in clinical and laboratory measures in a cohort of patients with rheumatoid arthritis. Study Design: Prospective, randomized and controlled trial in a sample of 37 patients with rheumatoid arthritis, assigned for omega-3 fatty acids supplements (n=11), mediterranean diet (n=8) and control group (n=15), during a 6 month period. The measures evaluated were number of tender and swollen joints, global health status (evaluated by the visual analogue scale), erythrocyte sedimentation rate, C-reactive protein, and the disease activity score DAS- 28 was obtained before and after the intervention. The statistical analysis of the results obtained was performed by means of the Wilcoxon and Kruskal-Wallis tests. Results: A significant reduction in the erythrocyte sedimentation rate was noted after 6 months of omega-3 fatty acids supplements. There was no significant variation in the remaining measures after 6 months, independently of the study group and the ongoing therapy for rheumatoid arthritis. However, a trend towards a reduction in DAS-28 and global health status measured by the visual analogue scale was noted. Conclusion: Omega-3 fatty acids supplements were associated with a reduction in the erythrocyte sedimentation rate after 6 months. The mediterranean diet had no impact in the measures evaluated. A trend towards a reduction in DAS-28 was observed in the group supplemented with omega-3 fatty acids, underscoring the need for further studies of these supplements as part of rheumatoid arthritis therapy, ideally, with bigger study samples. |
