Resumo: A prevalência de crianças e jovens com estilos de vida insuficientemente ativos é considerável, apesar dos evidentes benefícios da atividade física (AF) na saúde e das atuais linhas de recomendação. Estudos epidemiológicos e experimentais evidenciam relações entre a atividade física e a diminuição de mortalidade e morbidade, nomeadamente doenças cardiovasculares, perfil lipídico, densidade óssea, obesidade ou dor lombar inespecífica (DLI). Estudos recentes defendem que são vários os fatores de risco, que até recentemente não eram reconhecidos, a contribuir para o aumento da DLI na população infanto-juvenil. O objetivo do presente trabalho consiste em verificar: i) a prevalência de DLI em crianças e adolescentes e a relação entre a aptidão física (ApF) e a DLI (artigo I); ii) a associação entre o peso da mochila escolar, as variáveis antropométricas, a qualidade de vida relacionada com a saúde (QVRS) e a DLI (artigo II); iii) a associação entre o uso da mochila escolar, o pico de velocidade de crescimento (PVC), a QVRS e a DLI (artigo III). A amostra foi constituída por 149 alunos com idades compreendidas entre os 11 e os 17 anos provenientes de uma escola na cidade de Rio Branco/Acre, Brasil. Foi utilizada a Escala Visual Analógica (EVA), o Questionário Roland-Morris, o Teste de Sorensen, o Teste de Schober, a bateria Fitnessgram® e o Inventário Genérico PedsQL4.0TM. Também foi avaliado o peso da mochila escolar e as variáveis antropométricas. Os dados foram analisados com recurso a técnicas de comparação multivariada. Os resultados revelaram uma elevada presença de DLI com uma prevalência de 47.30%, das quais 60% foram identificadas como dor pontual. Entre os alunos do sexo feminino, verificou-se maior incapacidade funcional, menor QVRS nos domínios físico e emocional, nos resultados de saúde psicossocial, de saúde física e nos resultados totais do PedsQL; o grupo com DLI apresentou menor função física e menor saúde física, independentemente do sexo, idade e estatura; o peso da mochila escolar é semelhante em ambos os sexos, encontrando-se dentro dos valores recomendados; não foram encontradas diferenças significativas no Teste de Schober. A incapacidade funcional da dor lombar apresenta um aumento significativo no grupo pós-PVC, influenciado pelo sexo. O peso da mochila é significativamente superior no grupo pós-PVC. O teste de Sorensen foi significativamente menor no grupo pós-PVC. A QVRS apresentou níveis inferiores no grupo pós-PVC, exceto para a função escolar. Quanto à ApFRS: o grupo com DLI apresenta valores menores no teste de sentar-e-alcançar, na força/resistência abdominal, na extensão de tronco e vaivém, acompanhados por valores superiores no teste de Rolland-Morris. Deste modo, entre as principais conclusões destacam-se: (1) condições de DLI em crianças e adolescentes são comuns e apresentam alta incapacidade funcional principalmente nas raparigas; (2) há forte evidência entre ApFRS e a DLI; (3) o PVC está associado à menor percepção nos vários domínios da QVRS.
Despite the obvious benefits of physical activity (PA) in promoting health and current guidelines, the prevalence of children and young people with insufficiently active life styles is increasing. Epidemiological and experimental studies have shown a positive relationship between physical activity and the reduction of mortality and morbidity, acting to prevent the emergence of various diseases including cardiovascular disease, plasma lipid profile, bone density, obesity and nonspecific low back pain (NSLBP). New studies have suggested that there are several risk factors contributing to the increase of NSLBP in the juvenile population, which until recently were not recognized. The aim of this study is to verify: i) the prevalence of NSLBP in children and adolescents; the relationship of physical fitness (PFI) with the prevalence of NSLBP (Article I); ii) the association between the weight of the school backpack, anthropometric variables and quality of life related to health (HRQoL) with NSLBP (Article II); iii) the association between backpack use, growth velocity peak (GVP) and quality of life (QoL) with NSLBP (Article III). The sample consisted of 149 pupils aged between 11 and 17 years from a school in the city of Rio Branco/Acre, Brazil. It was used the Visual Analogue Scale (VAS) , the Roland-Morris Questionnaire, the Sorensen Test, the Schober’s Test, the battery of tests Fitnessgram® and the Generic Inventory PedsQL4.0TM It was also assessed the weight of the school bag and the anthropometric variables. All data were analyzed using multivariate comparison techniques. The results revealed a high presence of NSLBP with a prevalence of 47.30%, of which 60% were identified as occasional pain. In the female group was found a greater functional disability, lower QoL in physical and emotional domains, in psychosocial health score, in physical health score and also in the total score on the PedsQLTM; the group with NSLBP showed lower physical function and lower scores on physical health, regardless of gender, age and height; the weight of the school bag is similar in both sexes, lying within the recommended values; no significant differences were found in the Schober Test. the functional disability of low back pain presents a significant increase in the post-GVP group, being influenced by sex; backpack weight is significantly greater in the post-GVP group; the values obtained in the Sorensen Test were significantly lower in the post-GVP group; HRQoL showed higher levels in the pre-GVP group except for school function. Regarding HRPFI: the NSLBP group shows statistically lower values in the sit-and-reach test, in abdominal strength/endurance, trunk extension and the shuttle, along with higher values in the Rolland-Morris Test. Thus, among the main conclusions are: (1) conditions of NSLBP in children and adolescents are common and present high functional disability especially among girls; (2) there is strong evidence between HRPFI and NSLBP; (3) GVP is associated with lower perceptions in multiple domains of HRQoL.
