Tiroplastia tipo IV como cirurgia de reatribuição de género(M/F): caso clínico.

Abstract

Introdução A tiroplastia tipo IV consiste na aproximação cirúrgica da cartilagem cricoideia à cartilagem tiroideia, aumentando a força tensional das pregas vocais (frequência). O seu efeito simula a contração do músculo cricotiroideu. É efetuada no contexto de cirurgia de reatribuição de género masculino/feminino (M/F).No caso clínico em apreço, estamos perante uma doente submetida a tiroplastia tipo IV para reatribuição de género(M/F). A reatribuição de género (M/F) envolve também: terapia hormonal de substituição, cirurgia de feminização facial, mamoplastia de aumento, penectomia, vaginoplastia e condrolaringoplastia.Caso Clínico Descrevemos o caso de uma doente do sexo feminino, 31 anos de idade, com Disforia de género seguida na unidade de reconstrução génito-urinária e sexual (URGUS) desde 2019. Vem à consulta de Otorrinolaringologia por queixa de voz com frequência grave. Foi internada eletivamente em abril de 2022 para ser submetida a uma condroplastia da cartilagem tiroideia e tiroplastia tipo IV. As cirurgias não tiveram intercorrências e a doente teve alta sem queixas de relevo.Por último, comparámos em consultas de terapia da fala a folha de avaliação e reavaliação acústica e subjetiva pré e pós-operatória, tendo a doente mostrado satisfação com os resultados.ConclusãoNeste caso concluímos que a tiroplastia tipo IV é adequada para o aumento da frequência fundamental da voz (F0). A doente apresentava inicialmente F0 de 163Hz. A meio do 1º mês pós cirurgia tinha uma F0 de 203Hz. A morbilidade da cirurgia é baixa e o resultado é satisfatório e duradouro.

IntroductionType IV thyroplasty consists on the surgical approximation of the cricoid cartilage to the thyroid cartilage, increasing the tension force of the vocal folds (frequency). Its effect simulates the contraction of the cricothyroid muscle. It is performed in the context of androphony (Addison's disease, acromegaly), superior laryngeal nerve palsy, and male/female (M/F) gender reassignment surgery.In the present clinical case, we are dealing with a patient who underwent type IV thyroplasty for gender reassignment (M/F). Gender reassignment (M/F) also involves: hormone replacement therapy, facial feminization surgery, breast augmentation, penectomy, vaginoplasty and chondrolaryngoplasty. Case ReportWe describe the case of a female patient, 31 years old, with gender dysphoria, followed at the Genito-Urinary and Sexual Reconstruction Unit (GUSRU) since 2019. She comes to the Otorhinolaryngology consultation because of a high frequency voice complaint. She was admitted electively in April 2022 to undergo a thyroid cartilage chondroplasty and type IV thyroplasty. The surgeries were uneventful and the patient was discharged without major complaints.Finally, we compared the pre and postoperative acoustic and subjective assessment and reassessment sheet in speech therapy consultations, with the patient showing satisfaction with the results. ConclusionIn this case, we concluded that type IV thyroplasty is suitable for increasing the fundamental frequency of voice (F0). The patient initially had an F0 of 163Hz. In the middle of the 1st month after surgery she had an F0 of 203Hz. The morbidity of the surgery is low and the result is satisfactory and lasting.
