Relatório de Estágio e Monografia intitulada" Diabetes mellitus, doença de Alzheimer e respetiva possível relação bidirecional"

Abstract

The curricular internship is the culmination of a 5-year academic path. During the last semester of this Integrated Master in Pharmaceutical Sciences (MICF), carried out, in a first phase, a curricular internship in Community Pharmacy at Farmácia Santa Isabel, in Coimbra, supervised by Dr. Ana Rita Vale Rodrigues, between January 11th and April 30th, 2021; in a second phase, he does a curricular internship at Bluepharma, supervised by Dr. Diva Maria Silva, between May 3rd and July 30th, 2021. Both represented very important experiences in the consolidation of knowledge, as well as in the approximation from the MICF to the reality we find in the professional world. This document contains my reflection on this path in the form of a SWOT analysis.Diabetes mellitus and Alzheimer’s Disease are diseases which prevalence is increasing. On one side, Diabetes mellitus, for which the major signal is hyperglycemia and having multiple effects on many other levels, is clearly one of the major diseases in the 21st century. One the other side, Alzheimer’s Disease, being the primary cause of dementia in the elderly, and because its mechanisms are unclear, also deserves the attention from the scientific community.In the past few years several studies pointed to a possible connection between these two diseases. However, being one of them partially unknown, there is not yet a full knowledge on the specific links between Diabetes mellitus and Alzheimer’s Disease. At this point, there are several proposed mechanisms for this bidirectional relationship, as the damage on the blood-brain barrier (BBB), the appearance of central and peripheral inflammation, advanced glycation-end products (AGEs) and the mitochondrial dysfunction seem to assume a more prominent role.This monograph highlights the known mechanistic foundations of each disease in separate, as well as it intends to show possible links between them.

O estágio curricular é o culminar de um percurso académico de 5 anos. Ao longo deste último semestre no Mestrado Integrado em Ciências Farmacêuticas (MICF), realizei, numa primeira fase, estágio curricular em Farmácia Comunitária na Farmácia Santa Isabel, em Coimbra, orientado pela Dra. Ana Rita Vale Rodrigues, entre os dias 11 de janeiro e 30 de abril de 2021; numa segunda fase, realizei estágio curricular na Bluepharma, orientado pela Dra. Diva Maria Silva, entre os dias 3 de maio e 30 de julho de 2021. Tanto um como o outro, representaram experiências importantíssimas na consolidação de conhecimentos, bem como, na aproximação do MICF à realidade que encontramos no mundo profissional. No presente documento encontra-se a minha reflexão desse percurso sob a forma de análise SWOT (Strengths, Weaknesses, Opportunities, Threats).Tanto a Diabetes mellitus como a Doença de Alzheimer são patologias cujas prevalências apresentam tendências crescentes. Por um lado, a Diabetes mellitus, cujo sinal predominante é a hiperglicemia e com múltiplos efeitos em muitos outros níveis, assume-se claramente como uma das principais doenças do século XXI. Por outro lado, a Doença de Alzheimer, sendo a principal causa de demência em idosos, por não ver os seus mecanismos fisiopatológicos clarificados, merece, também, a maior atenção da comunidade científica. Nos últimos anos vários foram os estudos que apontaram para uma possível ligação entre estas duas doenças. Contudo, sendo pelo menos uma delas ainda parcialmente desconhecida, não há, ainda, um completo conhecimento das ligações concretas entre a Diabetes mellitus e a Doença de Alzheimer. Vários foram, já, os mecanismos propostos para esta relação bidirecional, sendo que o surgimento de danos na barreira hematoencefálica (BHE), o surgimento de inflamação central e periférica, os produtos resultantes de glicação avançada (AGEs) e a disfunção mitocondrial parecem assumir um papel de maior relevo. Esta monografia destaca as bases mecanísticas conhecidas de cada doença isoladamente, bem como pretende apontar as possíveis pontes e ligações entre elas.
