Relatórios de Estágio e Monografia intitulada “Novas Terapêuticas na Doença de Alzheimer”

Abstract

A doença de Alzheimer é uma doença neurodegenerativa e a forma mais comum de Demência, representando 80% dos casos. As terapêuticas já aprovadas baseiam-se essencialmente no alívio sintomático da doença, através dos inibidores da colinesterase, como o donepezilo, a galantamina e a rivastigmina, e de antagonistas dos recetores NMDA, como a memantina, não atuando diretamente na origem da patologia. Surgiu então a necessidade de procurar novas estratégias terapêuticas e novos alvos farmacológicos de modo a retardar a progressão da doença. Assim com o estudo dos biomarcadores da doença de Alzheimer, foram identificados possíveis alvos terapêuticos, como as placas de beta-amiloide e as tranças neurofibrilhares de proteína tau. Desenvolveram-se estratégias terapêuticas com alvo nestes biomarcadores, que passam essencialmente pela inibição da sua agregação e pela imunoterapia, com o uso de anticorpos monoclonais específicos contra estas estruturas. Com esta monografia pretendeu-se fazer uma revisão da literatura para sistematizar as várias estratégias terapêuticas que se encontram atualmente em investigação e os vários compostos em ensaios pré-clínicos e clínicos que são potenciais novas opções terapêuticas no tratamento da doença de Alzheimer.A Farmácia Comunitária é a face mais visível da profissão, onde o farmacêutico presta o melhor serviço nas farmácias em prol dos utentes, com o objetivo de satisfazer as necessidades dos mesmos e melhorar a saúde individual e coletiva da sociedade. É o primeiro local a que os cidadãos recorrem quando surgem questões relacionadas com a saúde, tornando-se assim num setor com uma importância estratégica no sistema de saúde, onde o farmacêutico assume um papel relevante na sociedade. O Estágio na Farmácia Vidigal permitiu adquirir bases para o futuro no mercado de trabalho, proporcionando o contacto direto com a realidade das farmácias e permitiu-me também executar diversas tarefas que nos compete enquanto farmacêuticos, sendo um complemento bastante enriquecedor na nossa formação. O circuito do medicamento incorpora em si diversas etapas, iniciando-se no fabrico e culminado, na maioria dos casos, na farmácia comunitária e no utente. A Distribuição interliga este circuito, sendo um setor fulcral e encarregando-se de fazer o medicamento chegar ao seu destino de forma eficiente e segura. O farmacêutico pode ter diversas funções e papéis nos vários departamentos existentes na Distribuição Grossista, tendo grandes responsabilidades nomeadamente a nível da Direção Técnica dos armazéns. O Estágio na Plural+Udifar permitiu ter uma perspetiva diferente e abrangente de como funciona uma distribuidora grossista, seguindo todo o percurso do medicamento desde que este chega ao armazém até ao seu destino.

Alzheimer’s disease is a neurodegenerative disease and the most common form of dementia, accounting for 80% of cases.The therapies approved so far are essentially based on the symptomatic relief of the disease, through cholinesterase inhibitors, such as donepezil, galantamine and rivastigmine, and NMDA receptor antagonists, such as memantine, not acting directly on the origin of the pathology.Then, came the need to search for new therapeutic strategies and new pharmacological targets to slow the progression of the disease. Thus, we studied the biomarkers of Alzheimer’s disease and recognized possible targets, such as beta-amyloid plaques and tau protein neurofibrillary tangles. Therapeutic strategies targeting these biomarkers have been developed, which essentially involve inhibition of their aggregation and immunotherapy, with the use of specific monoclonal antibodies against these structures.This monograph is intended to review the literature to systematize the various therapeutic strategies currently under investigation and the various compounds in pre-clinical and clinical trials that are potential new therapeutic options in the treatment of Alzheimer’s disease.The Community Pharmacy is the most visible face of the pharmaceutical profession, with the pharmacist providing, in the pharmacy, the best service for the patient, with the main point of satisfying their necessities and improving the individual and collective health of the society. The Pharmacy is the first local that people go to when a question related to health comes up, being a sector with a strategic relevance in the healthcare system, where the pharmacist highlights itself in the society with a greater importance. The Internship in Vidigal Pharmacy allowed me to acquire the basis for the future job market, allowing a direct contact with the reality of a pharmacy, and allowing me to do a wide range of tasks, being a complement to our formation. The Drug Circuit incorpores in itself diverse phases, beginning in the manufacturing and culminating, in most of the cases, in the community pharmacy and the patient. The Distribution interlinks this circuit, being a fundamental sector that provides the drugs to their destiny in an efficient and safe manner. The Pharmacist can do a lot of functions and roles in the diverse departments of the Grossist Distribution, having main responsibilities as in the specific case of the Technical Direction of the warehouses. The Internship in plural+udifar allowed me to obtain a wider and different perspective of the processes in the grossist distribution, following the circuit of the drugs from the moment they arrive in the warehouse until they leave for their destiny.
