Abstract: Introdução: Os feocromocitomas (FEO) e paragangliomas (PGL) metastáticos (FEO/PGLM) são definidos pela objetivação de metástases em locais não cromafins. Neste sentido, vários estudos foram realizados com intuito de identificar os fatores preditores de malignidade dos FEO/PGL, no entanto, verifica-se ausência de evidência científica sobre o comportamento tumoral dos mesmos. Até à data, não existem estudos em coortes portugueses sobre a temática. Pretendeu-se, assim, identificar potenciais fatores preditores de malignidade e de agressividade tumoral nos doentes com FEO/PGLM seguidos no Centro Hospitalar e Universitário de Coimbra.Materiais e Métodos: Foram analisados retrospetivamente 11 doentes com diagnóstico de FEO/PGLM, desde 1983 até ao final de 2020. Todos os doentes foram caracterizados na sua demografia, clínica, bioquímica, genética, histopatologia, imagiologia, terapêutica e sobrevivência. Para a avaliação de resposta à terapêutica foi utilizada a classificação Response Evaluation Criteria in Solid Tumours 1.1. Conceptualizou-se agressividade tumoral pela presença de doença progressiva, em resposta à terapêutica, ou morte por FEO/PGLM.Resultados: Evidenciou-se metastização em 8,64 anos (0-18 anos) após o diagnóstico do tumor primário (TP), observando-se uma maior incidência no género feminino e uma média de idades aquando diagnóstico do TP de 38,36 anos (12-72 anos). O perfil tumoral noradrenérgico correspondeu a doentes com metastização síncrona e diâmetro tumoral máximo (DTM) superior. Verificou-se elevação da cromogranina A síncrona com a existência de metastização. O DTM médio foi de 7,41 cm, onde 87,5% dos doentes apresentaram um DTM ≥ 5 cm. Mais de 70% demonstrou um Pheochromocytoma of the Adrenal Gland Scaled Score superior a 4. 36,36% doentes apresentaram maior agressividade tumoral, onde se incluíram os PGL, DTM superior e todos os doentes com metastização hepática. As curvas de Kaplan-Meier aquando o diagnóstico do TP e metastização demonstraram uma taxa de sobrevivência geral aos 15 anos de 90% e 57,1%, respetivamente.Discussão: Os preditores de malignidade elencados corroboram a evidência científica conhecida. Relativo à agressividade tumoral, é de interesse aprofundar o seu conceito. Nenhum estudo apresentou a definição de agressividade tumoral defendida. Acredita-se que todos os FEO/PGL apresentam potencial metastático, o que traduz um importante papel no seguimento do doente, devendo este ser alvo de uma maior reflexão nas instituições de saúde. Conclusão: Até ao momento nenhum marcador é 100% preditivo de metastização ou agressividade tumoral. Dadas as limitações do estudo, apela-se à implementação de um estudo multicêntrico e prospetivo com base nas características estudadas, nomeadamente no que concerne à investigação da agressividade tumoral.
Introduction: Metastatic pheochromocytomas (PHEO) and paragangliomas (PGL) (MPPGL) are defined by the objectivation of metastases in non-chromaffin tissue. Therefore, several studies were performed to identify PHEO/PGL predictors of malignancy, however there is a lack of scientific evidence about the tumoral behaviour. Until this date, there are no studies in Portuguese cohorts on the subject. Thus, it was intended to identify potential predictors of malignancy and tumour aggressiveness in MPPGL patients of the Centro Hospitalar e Universitário de Coimbra.Materials and methods: 11 patients with MPPGL diagnosis were analysed retrospectively between 1983 until end of 2020. All patients were characterized based on demographics, clinics, biochemistry, genetics, histopathology, imageology, therapeutics and survival. The Response Evaluation Criteria in Solid Tumours 1.1 classification was used to evaluate response to therapy. Tumour aggressiveness was conceptualized by the presence of progressive disease, in response to therapy, or death by MPPGL.Results: Metastasis was observed 8.64 years (0-18 years) after the diagnosis of the primary tumour (PT) with a bigger incidence in the female gender and a median age of PT diagnosis of 38.36 years (12-72 years). The noradrenergic tumour profile was seen in patients with synchronous metastases and a superior maximum tumour diameter (MTD). The elevation of chromogranin A was synchronous with the presence of metastases. Medium MTD was 7.41 cm, with 87.5% of the patients presenting MTD ≥ 5 cm. More than 70% demonstrated a Pheochromocytoma of the Adrenal Gland Scaled Score superior to 4. A higher tumoral aggressiveness was seen in 36.36% of the patients, which included PGL, superior MTD and all the patients with liver metastases. The Kaplan-Meier curves upon PT diagnosis and metastasis demonstrated an overall survival rate at 15 years of 90% and 57,1%, respectively. Discussion: The predictors of malignancy listed confirm the known scientific evidence. Concerning tumoral aggressiveness, it is of interest to deepen the concept. No other study presented the definition of tumour aggressiveness defended in this study. It is believed that all PHEO/PGL have metastatic potential, which translates into an important role in patient follow-up that should be the subject of further reflection in health institutions.Conclusion: As of right now, no marker is 100% predictive of metastasis or tumour aggressiveness. Given the limitations of the study, the implementation of a multicentre and prospective study based on the characteristics studied is recommended, namely regarding the investigation of tumour aggressiveness.
