dc.description.abstractIntrodução: A medicina centrada na pessoa (MCP) é um método clínico com evidência de gerar melhor relação entre o médico e o doente, capacitando-o e facilitando a adesão terapêutica. As intervenções em MCP, realizadas em médicos, mostram efeito na potenciação da sua aplicação na prática clínica e, por isso, há necessidade de a avaliar de forma disseminada, como neste tipo de formação nos cuidados de saúde primários em Portugal.Objetivos: Verificar a perceção dos pacientes de consulta segundo o modelo da MCP após intervenção formativa nos médicos, percebendo o impacto de tal formação em tempo de pandemia em duas Unidades de Saúde Familiar (USF) da região centro do país.Materiais e Métodos: Foi desenvolvido um estudo observacional em ciclo de garantia de qualidade, em amostra quasi-aleatória de pessoas que tiveram consulta de medicina geral e familiar (MGF), às quais se aplicou o questionário Patient Perception of Patient-Centeredness na versão portuguesa (PPPC-VP) por via telefónica em duas fases separadas por uma intervenção formativa aos médicos. Os questionários foram feitos a utentes da USF Topázio e USF Norton de Matos. Analisaram-se os resultados através de estatística descritiva e inferencial, bem como pela dinâmica de crescimento.Resultados: Foi estudada uma amostra de 129 utentes, a maioria do sexo feminino, com idade entre os 35 e os 64 anos, perfil socioeconómico de classe média e com seguimento habitual pelo médico com quem tiveram consulta. Entre as duas USF e ambos os tempos do estudo não foram encontradas diferenças significativas nas caraterísticas da amostra. A resposta mais prevalente ao questionário PPPC-VP foi a opção “Completamente”. Verificaram-se dinâmicas de crescimento positivas e negativas por pergunta entre os dois tempos de aplicação. Em cada USF a pergunta 9 foi aquela que mostrou maior dinâmica de crescimento, negativa na USF Topázio e positiva na USF Norton de Matos, com diferença significativa entre as USF. A média das pontuações totais dos questionários entre os dois tempos de estudo mostrou diferença estatisticamente significativa, mas não a pontuação de cada pergunta. A USF Topázio mostrou diferença significativa entre os tempos de aplicação do questionário, com uma pontuação média superior na segunda aplicação.Discussão: Os utentes, de forma geral, parecem percecionar as consultas destas USF como centradas na pessoa. As diferenças encontradas na pergunta 9 são explicáveis pelas discrepâncias de formação em MCP entre as duas USF. Os melhores resultados globais dos questionários na segunda fase de aplicação, após exposição aos médicos dos resultados da primeira fase, poderão reforçar a capacidade da intervenção formativa em potenciar a utilização da MCP.Conclusão: A investigação em MCP pode ter efeitos positivos, mesmo em contexto pandémico, e por isso deverão ser realizados estudos em amostras maiores e englobando mais USF.por
dc.description.abstractIntroduction: Patient-centered medicine (MCP) is a clinical method with evidence of creating a better relationship between doctor and patient, by empowering the latter and facilitating the therapeutic compliance. Interventions on MCP, when performed in physicians, show effect in improving its application in clinical practice, therefore, there is the need to disseminate the evaluation of this method, like in this kind of formation in the primary health care of Portugal.Objectives: Verify the patient’s perception of a consultation according to the MCP model, intervene formatively in doctors and understand the impact of this formation during a pandemic in two Family Health Units (USF) of the country’s central region.Materials and Methods: An observational and longitudinal study was carried out in a quasi-randomised sample in people that had a consultation in general and family medicine (MGF), to whom the questionnaire Patient Perception of Patient Centeredness in its portuguese version (PPPC-VP) was applied through phone call, in two phases separated by a formative intervention to physicians. The questionnaires were applied to patients of the USF Topázio and USF Norton de Matos. The results were analysed through descriptive and inferential statistical analysis as well as dynamic growth.Results: A sample of 129 patients was study, most of them female, aged between 35 and 64 years old, with a socioeconomical profile of middle class and with usual follow-up by the doctor with whom they had consultation. Between the two USF and both times of the study there was no significant differences in the sample’s features. The most prevalente answer of the PPPC-VP questionnaire was the option “Completely”. Positive and negative dynamic growths were verified between the two application times. In each USF the question 9 was the one that showed the greatest dynamic growth, negative in the USF Topázio and positive in the USF Norton de Matos, with significant difference between them. The average of the total scores of the questionnaires between the two times of study showed a statistically significant difference, but not the score in each question. The USF Topázio showed a significant difference between the times of application of the questionnaire, with a higher average acore in the second application.Discussion: Overall, the patients seem to perceive the consultations of these USF has patient-centered. The differences found in question 9 are suggested by the discrepancy in the MCP’s formation between the USF. The improvement of the global results of the questionnaires in the second phase of application, after the publication of the first phase results, may reinforce the hability of the formative intervention in spreading the application of MCP.Conclusion: Investigation in MCP may have a positive effect, even during a pandemic, therefore, studies with bigger samples and involving more USF should be performed.eng
