dc.description.abstractA disforia de género é uma entidade que tem merecido atenção crescente da sociedade civil, e consequentemente da Medicina, em particular da Psiquiatria, Sexologia e Endocrinologia. Nos últimos anos tem havido um aumento significativo das pessoas que procuram cuidados de saúde nesta área. Ainda assim não tem um foco global no ensino médico. Com base neste facto, efetuou-se uma revisão que abrange as alterações que tem sofrido nos últimos anos, os cuidados que os clínicos devem ter ao lidar com indivíduos transgénero, os seus critérios de diagnóstico, as terapêuticas disponíveis, assim como os seus efeitos secundários e a sua monitorização clínica, imagiológica e laboratorial, necessária para o seu seguimento adequado. De forma a obter a informação integrada nesta revisão foi feita uma pesquisa na base de dados da PubMed, incluindo publicações de todas as categorias em línguas portuguesa e/ou inglesa, datadas desde 2013 até 2020. Cada referência foi avaliada a partir do resumo de forma a selecionar, para leitura integral e posterior inclusão nesta revisão, os que mais concretamente abordassem a temática. A disforia de género, com as recentes mudanças no paradigma social, tem sofrido alterações no seu diagnóstico e nomenclatura, demonstrados pelas alterações propostas para a nova ICD-11, com implementação prevista em 2022. Também os princípios de base para gestão destes indivíduos têm sido alterados, adaptando as novas terminologias e cuidados específicos de forma a promover a sua saúde e bem-estar geral. As terapêuticas disponíveis podem ser implementadas em diferentes fases. O bloqueio hormonal da puberdade, tem a dupla função de minimizar a disforia de género na infância, assim como de estender a sua fase diagnóstica ao facilitar a transição social destes jovens. Para tal, são utilizados análogos da GnRH. Mais tarde, é feita a implementação da terapêutica hormonal de reatribuição sexual, utilizando testosterona para virilização ou estrogénios aliados a anti androgénios para feminização. Estas terapêuticas permitem a estes indivíduos finalmente expressar a sua identidade de género, beneficiando assim o seu prognóstico psicológico, social e o seu bem-estar geral. As presentes terapêuticas têm um bom perfil de segurança a curto e médio prazo, no entanto, não estão isentas de efeitos secundários. Destes destacam-se os efeitos cardiovasculares, nomeadamente, o risco de eritrocitose na terapêutica com testosterona, e o risco de eventos tromboembólicos na terapêutica com estrogénios. Ainda assim, com a melhoria nas formulações existentes aliada à monitorização recomendada, têm se tornado menos significantes. A consciencialização para a angústia sofrida por esta população e para as formas de a diminuir é essencial para a melhoria dos cuidados que necessitam.por
dc.description.abstractGender dysphoria has had an increased attention in the civil society, and therefore in Medicine, particularly in Psychiatry, Sexology and Endocrinology. Yet, it still does not have a global focus on medical school’s curriculum. Based on this fact, a review was carried out that covers the recent changes it has suffered, the special base care transgender individuals need, its diagnosis criteria, the types of therapy available, as well as the effects of said therapy and the supervision it needs. To obtain the information used in this article, a search was carried out in the PubMed database, which included publications of all categories published in Portuguese and/or English dated from 2013 to 2020. The selection of the most appropriate articles for posterior complete reading and inclusion in this review was carried out based on the reading of the abstract of each one. With the recent changes in the social paradigm, gender dysphoria has also suffered changes in its diagnosis and nomenclature. This is demonstrated by the proposed changes for the new ICD-11, that will be implemented in 2022. The base principles of care of these individuals have also been altered, adapting to the new terminologies and to the special care used to promote health and well-being. The available therapies can be implemented in different phases. The first available one is the puberty suppression therapy, used for both alleviating the gender dysphoria in childhood, as well as, extending its diagnostic phase with the easing of the social transition. To do so, it is used GnRH analogues. Later, the implementation of gender affirmation therapy is made, using testosterone for virilization or oestrogens allied with antiandrogens for feminization. These therapies allow transgender individuals to fully express their gender identity, and so, improving their psychological and social prognosis, as well as their well-being. Both types of therapies have a good security profile at short and medium term, yet they are not free of adverse effects. The cardiovascular effects, namely, the risk of polycythaemia with the therapy with testosterone, and the risk of thromboembolic events with the use of oestrogens, are the most common. Still, with the improvement of the formulations used in treatment, as well as the better control used, they have become less significant. Awareness for the distress suffered by this population and for the ways to alleviate said distress are essential to improve the medical care they need.eng
