Introduction: Overweight and childhood obesity have shown a prevalence with an increasing trend in the world and in parallel with the increase of the children’s age. These health problems as well as the associated comorbidities are largely preventable, and their prevention and treatment should be considered a priority based on an approach in terms of eating habits and physical activity. The parents are responsible for establishing habits in children, but the doctors have a unique role by influencing attitudes, beliefs, and behaviors. The counseling on physical activity in health services is one of the best strategies to increase the physical activity levels of populations. However, it is crucial to correctly assess overweight, obesity, habitual levels of physical activity, as well as provide recommendations and adopt appropriate therapeutic strategies. Objectives: [i] to summarize the main characteristics of childhood obesity; [ii] list the indications related to physical activity in children and adults, highlighting their health benefits; [iii] describe the methods of assessing physical activity; [iv] review the Portuguese panorama with regard to the assessment and monitorization of physical activity habits in Primary Health Care; [v] report the family variables that could affect physical activity habits in children and adolescents with overweight and primary childhood obesity; [vi] address intervention strategies in the prevention and treatment of childhood overweight and obesity. Methodology: A search was performed in electronic databases (PubMed®, SciELO, Embase, SCOPUS, ClinicalKey) based on the equation [(physical activity) AND (overweight OR pediatric obesity OR body weight changes) AND (family relations) AND (adolescent OR child *)] for English research and [(pediatric obesity OR overweight) AND physical exercise)] for Portuguese research, complemented by research for references, published between 2010 and 2020. Pediatrics books were consulted, and information, studies and recommendations were collected from: World Health Organization, Portuguese Directorate-General of Health, Portuguese National Health Service, Portuguese National Health Institute Doctor Ricardo Jorge, American College of Sports Medicine and United States Centers for Disease Control and Prevention. Conclusion: According to the sources there are differences in the classification of childhood obesity as well in the recommendations for the practice of physical activity, which are different according to the age. For the pediatric population there is no validated method for assessing physical activity to be applied by the general practitioner, similarly to that already exists for adults. Parents have an influence on their children's physical activity habits so the treatment and prevention of overweight and childhood obesity with family involvement should be the preferred strategy to be applied in future intervention programs which may be developed in the couple of years.eng
