dc.description.abstractO glaucoma é considerado a segunda causa mais comum de cegueira a nível mundial. Hoje em dia, apesar de ainda não existir cura nem forma de reverter a visão perdida, existem medicamentos capazes de atenuar e retardar a progressão da doença, como o caso dos colírios. Apesar da sua facilidade de aplicação, exigem uma administração sistemática ao longo do dia, onde apenas 5 % da dose instalada é retida pela córnea. Acrescenta-se o facto da necessidade de sucessivas instilações diárias, provocarem no paciente um incumprimento da posologia correta. Os sistemas de libertação controlada de fármacos, como os insertos oculares, surgem em prol da melhoria da eficácia do tratamento. Apresentam vantagens substanciais, permitindo manter o nível terapêutico adequado, a uma taxa de libertação controlada, durante um período de tempo definido. Desta forma, é maximizada a relação dose-eficácia, diminuindo consideravelmente as dosagens diárias necessárias.Um dos objetivos da presente dissertação foi avaliar a cinética de libertação do Cloridrato de Dorzolamida, através de um inserto ocular insolúvel, composto por copolímero de PCL/PEG (sintetizado segundo o mecanismo reacional de polimerização por abertura de anel). O fármaco é imobilizado homogeneamente na matriz polimérica, através de uma mistura física. O estudo da libertação baseou-se na avaliação do perfil cinético, durante 7 dias, quando os insertos foram submetidos em meios com diferentes valores de pH (6, 7, 8 e 9, de acordo com o pH do filme lacrimal). Os perfis apresentaram uma libertação superior a 90 % e de acordo com a Lei de Fick, excetuando o inserto inserido na solução com pH 9, onde após 24 horas se encontrava desfeito.Os polímeros e copolímeros sintetizados foram submetidos a uma análise RMN, onde se verificou uma proporcionalidade inversa entre a percentagem de PEG e o peso molecular final do copolímero, isto é, uma maior quantidade de PEG implica peso molecular do copolímero menor. Por fim, foi avaliada a interação entre os sistemas polímero-polímero e polímero- fármaco, nomeadamente a solubilidade do fármaco na matriz polimérica, através de uma análise térmica de DSC. Os resultados obtidos revelaram uma alteração no comportamento térmico individual dos homopolímeros, quando submetidos à interação química entre eles, sendo que uma maior quantidade de PEG, conduz a uma curva térmica do copolímero mais próxima do estado amorfo. O cloridrato de dorzolamida, apresentou ser uma estrutura amorfa, não sendo solúvel na matriz polimérica.por
dc.description.abstractGlaucoma is considered the second most common cause of blindness worldwide. Today, although there is still no cure or way to reverse vision loss, there are medications that can attenuate and slow the progression of the disease, such as eye drops. Despite their ease of application, they require systematic administration throughout the day, where only 5% of the installed dose is retained by the cornea. In addition, the need for successive daily instillations causes the patient not to comply with the correct dosage. Controlled drug release systems, such as ocular inserts, are being used to improve treatment effectiveness. They have substantial advantages, allowing the appropriate therapeutic level to be maintained, at a controlled release rate, for a defined period of time. In this way, the dose-efficacy relationship is maximized, considerably reducing the necessary daily dosages.One of the objectives of the present dissertation was to evaluate the release kinetics of Dorzolamide Hydrochloride, through an insoluble ocular insert, composed of PCL/PEG copolymer (synthesized according to the ring opening polymerization reaction mechanism). The drug is homogeneously immobilized in the polymeric matrix by physical mixing. The release study was based on the evaluation of the kinetic profile, during 7 days, when the inserts are submitted in media with different pH value (6, 7, 8 and 9, according to the pH of the tear film). The profiles showed a release higher than 90% and according to Fick's Law, except for the insert inserted in the solution with pH 9, which were disaggregated after 24 hours.The synthesized polymers and copolymers were subjected to NMR analysis, where an inverse proportionality between the percentage of PEG and the final molecular weight of the copolymer was found, i.e., a higher amount of PEG implies a lower molecular weight of the copolymer. Finally, the interaction between the polymer-polymer and polymer-drug systems, namely the solubility of the drug in the polymer matrix, was evaluated using a DSC thermal analysis. The results obtained revealed a change in the individual thermal behavior of the homopolymers when subjected to chemical interaction between them, with a greater amount of PEG, leading to a thermal curve of the copolymer closer to the amorphous state. The dorzolamide hydrochloride, showed to be an amorphous structure, not being soluble in the polymer matrix.eng
