dc.description.abstract | Introdução: O acidente vascular cerebral (AVC) é uma das principais causas de morte a nível mundial e a primeira causa de morte em Portugal. Esta patologia tem influência em vários níveis – físico, mental e socioeconómico – no quotidiano do doente e na comunidade onde está inserido. A depressão e ansiedade são problemas que podem advir desta doença cerebrovascular. Este estudo pretende compreender qual o impacto da depressão e da ansiedade na funcionalidade e qualidade de vida em doentes pós-AVC e se existe uma associação entre estes distúrbios.Materiais e métodos: O estudo observacional, prospetivo e transversal foi efetuado durante o primeiro trimestre do ano de 2020. Recrutaram-se doentes com idades compreendidas entre os 45 e os 80 anos, com um único episódio de AVC há pelo menos 6 meses, seguidos em consulta externa de Neurologia e/ou Medicina Física e Reabilitação no Centro Hospitalar e Universitário de Coimbra (CHUC) e no Centro de Medicina de Reabilitação da Região Centro-Rovisco Pais (CMRRC-RP). Foram excluídos doentes que pelas suas incapacidades não conseguiam dar resposta aos questionários. Para caraterizar a amostra foi implementado um questionário demográfico-clínico. A relação das diferentes componentes foi avaliada com os instrumentos métricos EQ-5D, Medida de Independência Funcional (MIF) e Escala de Ansiedade e Depressão Hospitalar (HADS) e determinado o coeficiente de correlação de Spearman.Resultados: Foi obtida uma amostra de 12 doentes, 7 do sexo masculino e 5 do sexo feminino. O estudo demonstrou uma correlação positiva, moderada e estatisticamente significativa entre a escala EQ-5D e as escalas MIF (rs(12)=0.673, p=0.017) e uma correlação negativa entre a EQ-5D e a HADS-D (rs(12)=-0.633, p=0.027). Observou-se uma correlação positiva, moderada e estatisticamente significativa entre a escala HADS-A e a escala HADS-D (rs(12)=0.661, p=0.019).Conclusão: Neste estudo, a funcionalidade e a depressão apresentam impacto na qualidade de vida, existindo uma associação entre a ansiedade e depressão. Para melhor compreensão desta influência são necessários estudos longitudinais que incluam os antecedentes pré-mórbidos dos participantes e compreendam a sua importância no pós-AVC. Paralelamente, sugere-se a criação de um instrumento métrico específico para a avaliação da ansiedade e depressão em doentes com sequelas de AVC, possibilitando um acompanhamento clínico adequado e promoção da qualidade de vida. | por |
dc.description.abstract | Introduction: Stroke is one of the leading causes of death worldwide and the first one in Portugal. This patology has influence in several levels – physical, mental and socio-economic – in a patient’s daily life and in his community. Depression and anxiety are problems that can arise from this cerebrovascular disease. This study aimed to understand the impact of depression and anxiety on functionality and quality of life in stroke survivals and whether there is an association between these disorders.Materials and methods: The observational, prospective and cross-sectional study was carried out during the first quarter of 2020. Were recruited outpatients of Neurology and/or Physical Medicine and Rehabilitation at the Hospital and University Center of Coimbra and the Rehabilitation Medicine Center of the Center Region-Rovisco Pais, aged between 45 and 80 years, with a single stroke episode occurred more than 6 months ago. Patients who were unable to answer the questionnaires, due to to their disabilities, were excluded. To characterize the sample, a demographic-clinical questionnaire was implemented. EQ-5D scale, Functional Independence Measure (FIM) and Hospital Anxiety and Depression Scale (HADS) were used to achieve the aim of this study and it were determined Spearman correlation coefficients between them.Results: A sample of 12 patients was obtained, 7 males and 5 females. It was observed a positive, moderate and statistically significant correlation between the scores of the EQ-5D scale and the total score of FIM scale (rs(12)=0.673, p=0.017) and a negative correlation between the scores of EQ-5D and HADS-D scale (rs(12)= -0.633, p=0.027). This study showed a positive, moderate and statistically significant correlation between the results of the HADS-A scale and the HADS-D scale (rs(12)=0.661, p = 0.019).Conclusion: In this study, functionality and depression have impact on quality of life, and there is an association between anxiety and depression. To better understand this influence in the post-stroke, longitudinal studies, that include the pre-morbid antecedents of the participants, are needed. In parallel, it is suggested to create a specific metric instrument for the assessment of anxiety and depression in patients with stroke sequelae, enabling an adequate clinical follow-up and promotion of quality of life. | eng |
