A Diabetes Gestacional e o seu impacto na depressão e ansiedade da grávida

Abstract

Introdução: Existe um interesse cada vez maior na associação entre diabetes gestacional (DG) e a saúde mental da mulher no pré e pós-parto. Dada a divergência de resultados existente na literatura atual, este estudo pretende analisar uma amostra de grávidas com e sem DG da cidade de Coimbra, para avaliar a possível relação entre depressão e ansiedade e o diagnóstico de DG. Métodos: Estudo de coorte, analítico e retrospetivo. A amostra englobou 308 grávidas no 3º trimestre de gravidez seguidas nas Maternidades Bissaya Barreto e Daniel de Matos, que responderam a um questionário constituído por: avaliação sociodemográfica, perguntas diretas relativas a dados clínicos da grávida: gestações, existência de DG ou DM e a sua terapêutica, sintomas e tratamento de ansiedade, depressão ou doenças prévias, escala de rastreio de depressão pós-parto (ERDP-24) e escala de rastreio da ansiedade perinatal (ERAP). Realizou-se uma análise estatística descritiva e inferencial para comparar os grupos com e sem DG em relação aos níveis de depressão e ansiedade na gravidez.Resultados: A amostra incluiu 110 grávidas com DG e 198 grávidas sem DG. Tanto a análise global como a análise realizada por subgrupos, em que foram ajustadas algumas possíveis variáveis de confundimento, não mostraram uma relação significativa entre o diagnóstico de DG e um risco aumentado para depressão e ansiedade.Discussão e conclusão: Este estudo foi limitado pela utilização de escalas de autopreenchimento, pela elevada prevalência de grávidas com outras comorbilidades e pela representação insuficiente de certos subgrupos, como o caso de pessoas com níveis socioeconómicos mais baixos.

Introduction: There has been an increasing interest in the association between gestational diabetes (GD) and the mental health of women in the pre and postpartum period. Given the divergence of results in the current literature, this study aims to analyze a sample of pregnant women with and without GD from the city of Coimbra, to assess the possible relationship between depression and anxiety and the diagnosis of GD. Methods: Cohort, analytical and retrospective study. The sample comprised 308 pregnant women in the 3rd trimester of pregnancy followed at Bissaya Barreto and Daniel de Matos Maternities, who answered a questionnaire consisting of: a sociodemographic evaluation, direct questions related to clinical data of the pregnant woman: pregnancies, existence of GD or DM and their therapy, symptoms and treatment of anxiety, depression or previous illnesses, a postpartum depression screening scale (ERDP-24) and a perinatal anxiety screening scale (ERAP). A descriptive and inferential statistical analysis was performed to compare the groups with and without GD in relation to the levels of depression and anxiety during pregnancy.Results: The sample included 110 pregnant women with GD and 198 pregnant women without GD. Both the overall analysis and the analysis performed by subgroups, in which there was an adjustment of some possible confounding variables, did not show a significant relationship between the diagnosis of GD and an increased risk for depression and anxiety.Discussion and conclusion: This study was limited by the use of self-filling scales, by the high prevalence of pregnant women with other comorbidities and by the insufficient representation of certain subgroups, such as people with lower socioeconomic levels.
