dc.description.abstractIntrodução: A imagem corporal constitui um conceito multifacetado e em doentes com cancro da mama está positivamente relacionada com a qualidade de vida. Apesar do sucesso terapêutico nas sobreviventes, a perceção negativa acerca da sua própria imagem corporal pode constituir um precursor de mal-estar psicológico. O objetivo do presente estudo é analisar o impacto de algumas variáveis sociodemográficas, clínicas e psicológicas da própria doença na perturbação da imagem corporal num grupo de mulheres com antecedentes de cancro da mama submetidas a mastectomia unilateral.Material e Métodos: Este estudo observacional, transversal e descritivo incluiu 30 doentes com cancro da mama, submetidas a mastectomia unilateral. Foram selecionadas doentes do sexo feminino, com diagnóstico de cancro da mama em estadio Tis ou T1, N0, M0, submetidas a mastectomia unilateral, seguida ou não de hormonoterapia, e excluídas doentes que apresentassem comorbilidades major (condição médica ou psiquiátrica) ou que tivessem sido submetidas a tratamentos adjuvantes suscetíveis de influenciar as respostas ao inquérito, tais como quimioterapia ou radioterapia. Foram, igualmente, excluídas as doentes submetidas a cirurgia de reconstrução mamária ou que apresentassem linfedema do membro superior como consequência da mastectomia total com esvaziamento axilar. Com vista a avaliar a imagem corporal, foi usada a versão portuguesa da “Body Image Scale”. Foram recolhidos dados sociodemográficos e clínicos através do uso de uma grelha de informação elaborada pelos investigadores.Resultados: Verificaram-se diferenças significativas nas questões da “Body Image Scale” em algumas variáveis sociodemográficas e clínicas, entre elas a idade, o estado civil, o tempo decorrido desde a cirurgia e o nível socioeconómico na população do estudo. Para além disso, também as variáveis relacionadas com o impacto psicológico revelaram diferenças significativas: ansiedade em relação às doenças da mama; sentimento de ameaça à vida e/ou integridade física na data do diagnóstico; reação de medo intenso face ao diagnóstico.Discussão e conclusão: Os resultados obtidos suportam a hipótese de que determinados fatores sociodemográficos e clínicos, tais como a idade, o tempo decorrido desde a cirurgia, e a situação profissional, têm impacto nas diferentes vertentes da imagem corporal da mulher submetida a mastectomia após diagnóstico de cancro da mama. De forma a reduzir o impacto negativo que o cancro da mama tem na qualidade de vida destas mulheres, torna-se imperativo o investimento nesta área e possibilitar o acesso universal a reconstrução mamária imediata após cirurgia.por
dc.description.abstractIntroduction: Body image is positively related with quality of life in patients with breast cancer. Despite the therapeutic success regarding mortality rate, the negative perception of their own body image can cause psychological distress in these patients. The aim of this study is to analyse the influence of sociodemographic, clinical and psychological impact of the disease in the disturbance of body image in a group of women diagnosed with breast cancer who underwent unilateral mastectomy.Material and Methods: This observational, cross-sectional and descriptive study included 30 breast cancer patients who underwent unilateral mastectomy. Female patients, diagnosed with breast cancer in stage Tis or T1, N0, M0, submitted to unilateral mastectomy, either followed by hormone therapy or not were selected. Patients with major comorbidities (medical or psychiatric condition) or who had been subjected to adjuvant treatments that may influence their responses to the survey, such as chemotherapy or radiation therapy, were excluded. Patients who underwent breast reconstruction surgery or who had lymphedema of the upper limb as a result of total mastectomy with axillary dissection were also excluded. In order to assess body image, the Portuguese version of the “Body Image Scale” was used. Sociodemographic and clinical data were collected through the use of an information grid developed by researchers.Results: The results revealed significant differences in several questions on the “Body Image Scale” with some sociodemographic and clinical variables, including age, marital status, time since surgery and socioeconomic level. In addition, the variables related to psychological impact also revealed significant differences as anxiety in relation to breast diseases; feeling of threat to life and / or physical integrity at the time of diagnosis; reaction of intense fear when diagnosed.Discussion and Conclusion: The results obtained support the hypothesis that certain sociodemographic and clinical factors, such as age, time since surgery and professional situation, have an impact on the different aspects of a woman's body image after mastectomy. In order to reduce the negative impact that breast cancer has on the quality of life of these women, becomes imperative the investment on this subject and the global access to immediate breast reconstruction after surgery.eng
