Abstract: Introdução: O período perinatal é uma fase de vulnerabilidade para o aparecimento ou recorrência de perturbações de ansiedade, as quais afetam cerca de 15% das mulheres no período do pós-parto. A inexistência de um instrumento de rastreio da Ansiedade no pós-parto, validado para a população portuguesa, dificulta a precoce identificação, e consequente acompanhamento médico, destas mulheres. De acordo com o que sabemos, existe apenas um instrumento de rastreio específico para esta fase particular, a Escala de Rastreio da Ansiedade Perinatal (ERAP) com quatro dimensões: 1 - Preocupação excessiva/Medos específicos; 2 - Perfecionismo/Controlo/Trauma; 3 - Ansiedade Social; 4 - Ansiedade aguda e ajustamento.Objetivo: Analisar as propriedades psicométricas e operativas da ERAP no pós-parto (PPt).Métodos: 305 mulheres entre o 2º e 6º mês PPt (média de semanas do bebé=16.15±8.04) responderam à ERAP, à Escala de Rastreio da Depressão Pós-parto (ERPD-21) e ao Profile of Mood States (PoMS). Todas as participantes tinham respondido à ERAP no 2º trimestre de gravidez. Desta amostra, 188 (61.6%) participantes foram também entrevistadas com a Diagnostic Interview for Psychological Distress (DIPD) aproximadamente no sexto mês PPt.Resultados: A análise fatorial confirmatória do modelo de segunda ordem, baseado na versão original e previamente replicado com uma amostra de grávidas (n=606), revelou um ajustamento aceitável (X²/gl=2.4080; CFI=.9010; GFI=.8290; RMSEA=.0770), com a eliminação de um item com loading <.40 e a correlação de 23 pares de erros. Os coeficientes alfa de Cronbach da ERAP-30 e das suas quatro dimensões foram >.80. Os coeficientes de correlação da ERAP com a ERDP e com o Afeto Negativo e Positivo da PoMS foram significa-tivos e moderados. Os coeficientes de correlação entre as pontuações total e dimensionais na ERAP-30 aos 2 e aos 6 meses PPt foram significativos e elevados (r>.50; p<.01). As mu-lheres com perturbações de ansiedade (n=10; 5.3%) apresentaram pontuações totais e dimensionais significativamente superiores na ERAP-30 (todos p<.001). A análise de curvas ROC mostrou que o ponto de corte que potenciou o Yoden Index (.866) foi o 30, associado a uma sensibilidade de 90.00% e especificidade de 96.61%. A área abaixo da curva foi .972 (p<.001).Conclusão: A versão portuguesa da ERAP para o pós-parto revelou boa validade de construto, convergente-divergente, concorrente (utilizando como critério o diagnóstico) e fidelidade (consistência interna e estabilidade temporal), e excelente precisão para o rastreio de per-turbações de ansiedade. Vem colmatar a falta de um instrumento de rastreio específico para a ansiedade perinatal, podendo começar a ser utilizada para esse fim.
Introduction: Perinatal period is a phase of vulnerability for new onset or recurrence of an anxiety disorder, which is experienced by approximately 15% of women during post-partum. The non-existence of a screening instrument for assessing postpartum anxiety in the Portuguese population can lead to a delay in the detection and treatment of these women. To our current knowledge, there’s only one specific screening instrument for this particular period, ERAP, which evaluates four dimensions: 1 - Excessive Worry/Specific fears; 2 - Perfectionism/Control/Trauma; 3 - Social Anxiety; 4 - Acute anxiety and adjustment.Objectives: To analyze the psychometric and operative properties of ERAP for postpartum women.Method: 305 women, between the 2nd and 6th month postpartum (mean weeks of baby=16.15±8.04) answered a set of self-reported questionnaires containing ERAP, ERDP-21 and PoMS. All participants had answered ERAP in the 2nd trimester of pregnancy. From this sample, 188 participants also answered to a diagnostic interview around the 6th month post-partum.Results: The factorial structure of the second order model, based on the original version previously replicated with a sample of pregnant women (n=606), revealed an acceptable adjustment (X²/gl=2.4080; CFI=.9010; GFI=.8290; RMSEA=.0770), after the elimination of one item with a loading lower than .40 and correlating 23 pairs of errors. Cronbach’s alpha scores of ERAP-30 and its 4 dimensions were >.80. The correlation’s coefficients of ERAP with PDSS and with the Negative Affect and Positive Affect of PoMS were significant and moderate. In the study of temporal stability, the correlation between total and dimensional scores between the 2nd and 6th postpartum were significant and high (r>.50; p<.01). Women with anxiety disorders (n=10; 5.3%) presented significantly higher total and dimensional scores in ERAP-30 (all p<.001). ROC analysis showed that the cut-off point that potentiated the Yoden Index (.886) was of 30, associated with a sensitivity of 90.00% and a specificity of 96.61%. Area under curve was .972 (p<.001).Conclusion: The Portuguese version of ERAP for the postpartum revealed good construct validity, convergent-divergent validity, concurrent validity (using diagnostic as a criterion) and reliability, as well as excellent accuracy in screening for anxiety disorders. This scale fills the gap with regard to the lack of screening instrument for anxiety in postpartum and it is ready to be used for this purpose.
