Abstract: Prolonged hospitalizations phenomenon in the wards of hospitals with differentiated attention from the National Health System reveals itself to be a complex problem in contemporary society, which the Social System does not ignore. Despite such a relevance there is no consensus view accompanied by quantitative data that will allow us to act on this problem.Given the importance of this topic and in order to contribute to clarifying the prolonged hospitalization phenomenon, we tried to analyze the social nature's reasons that justify them in the differentiated care provided by the National Health System. The general objective of the study was to analyze the reasons for the prolonged hospitalizations of patients flagged for Social Department, during one year, in a Group I Hospital Center, belonging to the National Health System. An exploratory investigation was chosen. given the need for a better understanding of the phenomenon and a mixed methodology. The study was retrospective and descriptive, using data collection through documentary research. This study started by analyzing the 1239 patient cases referred to Social Department in 2018 and the sample under study was comprised of 155 users who experienced hospitalizations over 30 days. The results reveal that the users who experience prolonged hospitalizations belong to an aging and dependent population in the AVDs, who are mostly waiting for a response by RNCCI, further adding the failure to plan discharge and the lack of vacancies and institutional responses in the community. It is noteworthy that prolonged hospitalizations are justified by a number of factors, namely, the late signaling to Social System and the slowness of institutional responses in the community appropriate to the provision of care after hospital discharge.
