Abstract: The induction of a specific immune response to allergens, by subcutaneous or sublingual administration of allergen fragments or strata, has proved to be an effective strategy in the treatment of type I allergic reactions.However, associated with this strategy, adverse reactions have been observed, some quite serious, such as anaphylactic shock. For this reason, the group of people available and receptive to this therapeutic strategy is quite small.The scientific community is aware that it is necessary to develop new tools, in order to try to control the number of allergic patients that has been increasing in recent years.One of the strategies under development is genetic vaccines, which include DNA vaccines and messenger RNA (mRNA) vaccines. This pharmaceutical technology is based on the synthesis of the allergen in situ using the individual's cellular machinery, allowing the transcription and translation of the DNA or mRNA sequences originating the target allergen.Therefore, genetic vaccines provide the human organism with the possibility of synthesizing the allergen in a pure state and in a controlled quantity. This strategy has demonstrated the ability to modulate the immune response to allergens, namely, redirecting the response to a more balanced response in order to increase the production of type 1 helper T cells (Th1) and decrease type 2 helper T cells (Th2), which were initially involved in allergic reactions.The induction of this type of immune response in the body, achieved with the use of genetic vaccines, meets the typical immune response of a non-allergic individual, so genetic vaccines may be an important strategy to control type I allergic reactions over time.Thus, this monograph aims to provide an insight into the potential of genetic vaccines in combating allergies, as well as to present what is currently under development and clinical trials in this area.
A indução de uma resposta imunológica específica aos alérgenos, através da administração por via subcutânea ou sublingual, de fragmentos ou estratos dos alérgenos tem demonstrado ser uma estratégia eficaz no tratamento de reações alérgicas do tipo I. No entanto, associado a esta estratégia, tem-se observado o aparecimento de reações adversas, algumas bastante graves, como o choque anafilático. Por esta razão, o grupo de pessoas disponíveis e recetivas a esta estratégia terapêutica é bastante reduzida. A comunidade científica está consciente que é necessário desenvolver novas ferramentas, com o objetivo de tentar controlar o aumento do número de doentes alérgicos que se tem vindo a assistir nos últimos anos. Uma das estratégias em grande desenvolvimento são as vacinas genéticas onde se incluem vacinas de DNA e as vacinas de RNA mensageiro (mRNA). O fundamento desta tecnologia farmacêutica baseia-se na síntese do alérgeno in situ utilizando a maquinaria celular do indivíduo, permitindo a transcrição e tradução das sequências de DNA ou mRNA originando o alérgeno alvo. Assim sendo, as vacinas genéticas fornecem ao organismo humano a possibilidade de este sintetizar o alérgeno num estado puro e em quantidade controlada. Esta estratégia tem demonstrado capacidade de modular a resposta imunológica aos alérgenos, nomeadamente, redirecionando a resposta para uma resposta mais balanceada no sentido de aumentar a produção de células T helper do tipo 1 (Th1) e diminuir as células T helper do tipo 2 (Th2), células inicialmente envolvidas nas reações alérgicas. A indução deste tipo de resposta imune no organismo, conseguido com o recurso às vacinas genéticas, vai ao encontro da resposta imune típica de um individuo não alérgico, pelo que as vacinas genéticas poderão vir a ser uma estratégia importante no controlo de reações alérgicas do tipo I ao longo do tempo. Deste modo, esta monografia tem por finalidade fornecer uma visão sobre o potencial das vacinas genéticas no combate às alergias, bem como apresentar o que está, atualmente, em desenvolvimento e em ensaios clínicos nesta área.
