Abstract: Objetivo: O baricitinib, uma das mais recentes alternativas terapêuticas para doentes com artrite reumatoide (AR), é um inibidor seletivo da janus quinase (JAK) – JAK1 e JAK2. O presente trabalho teve por objetivo a realização de uma meta-análise de forma a avaliar o perfil de segurança do baricitinib nas doses de 2 e 4 mg, doses atualmente comercializadas. Métodos: Recorreu-se à base de dados PubMed para efetuar a pesquisa bibliográfica. Foi utilizado o programa RevMan versão 5.3 para calcular o risco relativo (RR) e intervalos de confiança de 95% (ICs) para todos os resultados ao longo da meta-análise, usando o modelo dos efeitos aleatórios Mantel-Haenszel. Resultados: Foram selecionados cinco ensaios clínicos randomizados que avaliaram a segurança do baricitinib em doentes com AR, efetuados numa população adulta e com a presença de grupo controlo. O baricitinib 2 mg demonstrou não aumentar os eventos adversos (RR: 1,01; 95% IC: 0,92-1,11; p=0,80), os eventos adversos graves (RR: 0,69; 95% IC: 0,38-1,25; p=0,22), as infeções graves (RR: 0,97; 95% IC: 0,28-3,37; p=0,96) nem o herpes zoster (RR: 2,32; 95% IC: 0,27-19,93; p=0,44). O baricitinib 4 mg também não fez aumentar os eventos adversos (RR: 1,08; 95% IC: 0,95-1,23; p=0,26), os eventos adversos graves (RR: 1,12; 95% IC: 0,77-1,63; p=0,56) nem as infeções graves (RR: 0,94; 95% IC: 0,47-1,90; p=0,86). No entanto, existiu uma tendência de aumento de herpes zoster (RR: 3,81; 95% IC: 1,3510,71; p=0,01) comparativamente ao placebo. Conclusão: O balanço final dos resultados obtidos é positivo, não existindo de um modo geral grandes discrepâncias entre os grupos alvos de estudo. Contudo, importa destacar a incidência significativa de herpes zoster no grupo baricitinib 4 mg.
Objective: Baricitinib, one of the most recent therapeutic alternatives for patients with rheumatoid arthritis (RA), is a selective inhibitor of janus kinase (JAK) - JAK1 and JAK2. The present study aimed to carry out a meta-analysis in order to assess the safety profile of baricitinib in doses of 2 and 4 mg, currently marketed doses.Methods: The PubMed database was used to perform the bibliographic search. The program RevMan version 5.3 was used to calculate the relative risk (RR) and 95% confidence intervals (CIs) for all results throughout the meta-analysis, using the Mantel-Haenszel random effects model. Results: Five randomized clinical trials were selected that evaluated the safety of baricitinib in RA patients, carried out in an adult population and with the presence of a control group. Baricitinib 2 mg has been shown not to increase adverse events (RR: 1.01, 95% CI: 0.92-1.11, P = 0.80), serious adverse events (RR: 0.69, 95% CI: 0.38-1.25, P = 0.22), severe infections (RR: 0.97, 95% CI: 0.28-3.37, P = 0.96) nor herpes zoster (RR: 2.32, 95% CI: 0.27-19.93, P = 0.44). Baricitinib 4 mg also did not increase adverse events (RR: 1.08, 95% CI: 0.95-1.23, P = 0.26), serious adverse events (RR: 1.12, 95% CI: 0.77-1.63, P = 0.56) nor serious infections (RR: 0.94, 95% CI: 0.47-1.90, P = 0.86). However, there was an upward trend in herpes zoster (RR: 3.81, 95% CI: 1.35-10.71, P = 0.01) compared to placebo. Conclusion: The final balance of the results is positive, with no general discrepancies between the target groups. However, it is important to highlight the significant incidence of herpes zoster in the baricitinib 4 mg group.
