Abstract

Folowed by a brief conslusion of the topic. Parte 3: Introduction: The definition of high-risk medication is not consensual among authors. However, every single one of them regards this type of medication as having an augmented risk of potentially causing harm to the user in comparison to other therapeutic options. They are regularly referred to in literature but mostly in a hospital setting. This is an outcome of the reduced number of studies involving high-risk medication in an ambulatory setting.Objective: Establish a list of high-risk medications for the ambulatory setting.Methods: Through research done in the Pubmed database between November 2019 and June 2020 with the following terms and combinations: polypharmacy, polimedication, potentially inappropriate medication, high-risk medication, high-alert medication, medication errors, adverse drug events, adverse drug reactions, ambulatory care. Related articles were searched. Consultation of high-risk medication lists and lists referring to the drugs more commonly associated with medication errors and adverse drug reactions.Results and conclusion: By the end, a new list of high-risk medication for the ambulatory setting was conceived. It is grounded on pharmacological bases that support the inclusion of all the admitted medications on such a list. Recommendations regarding approaches to reduce the potential harm caused to the patient were also added. The pharmacist is responsible and must have an active role to prevent and minimize the harm caused to a patient when this type of medication is being used.

Parte 3: Introdução: A definição de medicamentos de alto risco não é consensual entre os autores, porém todos consideram estes medicamentos como sendo medicamentos que acarretam um risco acrescido para o doente, comparativamente a outras opções terapêuticas. São várias vezes referenciados na literatura, mas o contexto em que surgem é quase sempre o hospitalar, sendo escassos os estudos que tratam os medicamentos de alto risco em meio ambulatório. Objetivo: Elaboração de uma lista de medicamentos de alto risco em contexto de ambulatório.Métodos: Pesquisa na PubMed entre novembro de 2019 e junho de 2020 para os seguintes termos e combinações: polypharmacy, polimedication, potencially inapropriate medication, high-risk medication, high-alert medication, medication errors, adverse drug events, adverse drug reactions, ambulatory care. Pesquisa de artigos relacionados. Consulta de listas de Medicamentos de Alto Risco e de listas referentes aos fármacos mais frequentemente responsáveis por erros de medicação ou por reações adversas medicamentosas.Resultados e Conclusão: Obteve-se uma nova lista de medicamentos de alto risco devidamente fundamentada com as bases farmacológicas que justificam a inserção destes medicamentos numa lista deste tipo, indicando-se as principais recomendações para minimizar o dano, em contexto de ambulatório. O farmacêutico pode e deve ter um papel ativo na prevenção e contenção do dano no doente provocado por estes medicamentos.
