dc.description.abstractHeart Failure (HF) is one of most deadly and disabling cardiovascular diseases and its incidence is increasing steadily with the ageing of the western population. Adherence to antihypertensive therapy has a major influence on the outcomes of heart failure, not limited only to the treatment of high blood pressure, but also the regulation of the hydro-electrolytic balance and the reduction of cardiac workload. In order to achieve better outcomes, the identification of the main reasons underlying the low adherence rates to optimized HF medical therapy will allow us to develop more individualized strategies.This study aimed to evaluate heart failure patients' adherence to antihypertensive therapy and associated causes for non-adherence. We also intended to compare the scores obtained within the two defined study arms – hypertensive (HT) and hypotensive (HoT)- within the HF group. METHODSCross-sectional study conducted in the cardiology department of Coimbra´s Tertiary Hospital. Between June 2019 and November 2019 all consecutive adult heart failure patients attending to the outpatient HF outpatient clinic, with a prescription of, at least, one antihypertensive drug, were invited to participate. This study was approved by the local Ethics Commission (CE 008-19).Socio-demographic and patient-related data were collected and the short version of Maastricht Utrecht Adherence in Hypertension (MUAH-16) was applied. Statistical analysis was performed using the IBM SPSS® Statistics software. Statistical analysis was performed using the IBM SPSS® Statistics software. A total of 125 patients were enrolled in the study, 72.8% males, with a mean age of 65.7 ± 1.7 years (62 HT + 63 HoT). The patients who presented with normal blood pressure were assigned to the HT group.The mean score of MUAH-16 on the total sample was 93.4 ± 7. 5 (in a possible range of 16 to 112), with no statistically significant differences between HT and HoT patients. Considering the mean scores of MUAH-16 subscales, being 28 points the maximum scoring, the results obtained were: positive attitude toward health care and medication: 25.9 ± 2.6; lack of discipline: 25.4 ± 3.5; aversion toward medication: 19.7 ± 4.7, and active coping with health problems: 22.1 ± 3.3. Sub-group analysis shows no statistically significant differences between HT and HoT patients.The studied population showed satisfactory levels of adherence to antihypertensive therapy, even so, improvements can be obtained if specific strategies to decrease aversion toward medication and to improve active coping with health problems are developed.eng
dc.description.abstractA Insuficiência Cardíaca é uma das patologias cardiovasculares mais mortal e debilitante e, a sua incidência, está a aumentar a um passo constante de acordo com o envelhecimento da população ocidental.A adesão à terapêutica antihipertensora tem uma grande influência no decurso da doença, não se limitando apenas ao tratamento da pressão alta, mas também à regulação do equilíbrio hidroeletrolítico e à redução da sobrecarga cardíaca. Para alcançar melhores resultados, a identificação dos principais motivos subjacentes às baixas taxas de adesão à terapêutica farmacológica em doentes diagnosticados com insuficiência cardíaca, permitirá uma otimização destes valores através do desenvolvimento de estratégias individualizadas.Este estudo teve como objetivo avaliar a adesão à terapêutica antihipertensora em doentes com insuficiência cardíaca e as causas associadas à não adesão. Pretendemos igualmente comparar as pontuações obtidos nos dois braços definidos - hipertensos (TH) e hipotensores (HoT) - dentro do grupo de participantes diagnosticados com Insuficiência Cardíaca.Materiais e MétodosEstudo clínico observacional transversal realizado no Serviço de Cardiologia do Centro Hospitalar e Universitário da Universidade de Coimbra. Entre junho de 2019 e novembro de 2019, todos os adultos da consulta externa, com diagnóstico de Insuficiência Cardíaca , e prescrição de pelo menos um medicamento antihipertensor, foram convidados a participar. Este estudo foi aprovado pela Comissão de Ética local (CE 008-19). Os dados sociodemográficos e relacionados com o estado de saúde do doente foram recolhidos e o questionário Maastricht Utrecht Adherence in Hypertension (MUAH-16) foi aplicado. A análise estatística foi realizada usando o software IBM SPSS® Statistics. Foram incluídos no estudo 125 doentes, 72,8% do sexo masculino, com idade média de 65,7 ± 1,7 anos (62 HT + 63 HoT). Os doentes que apresentaram pressão arterial normal foram considerados para o grupo HT.A pontuação média do MUAH-16 na amostra total foi de 93,4 ± 7,5 (em uma faixa possível de 16 a 112 pontos), sem diferenças estatisticamente significativas entre os doentes com TH e HoT. Considerando as pontuações médias das subescalas MUAH-16, sendo 28 pontos a pontuação máxima, os resultados obtidos foram: atitude positiva em relação à assistência médica e medicamentos: 25,9 ± 2,6; falta de disciplina: 25,4 ± 3,5; aversão à medicação: 19,7 ± 4,7 e enfrentamento ativo de problemas de saúde: 22,1 ± 3,3. A análise de subgrupos demonstra que não existem diferenças estatisticamente significativas entre os pacientes com TH e HoT. A população em estudo apresentou níveis satisfatórios de adesão à terapia antihipertensora. Apesar deste facto, melhorias podem ser obtidas se forem desenvolvidas estratégias específicas para diminuir a aversão à medicação e melhorar o coping ativo com os problemas de saúde.por
