Abstract: Objetivo: Caracterizar os estadios iniciais da retinopatia diabética (DR) e identificar, num grupo de doentes com diabetes tipo 2 sem DR ou com DR ligeira, preditores de progressão para edema macular envolvendo o centro (CIME), num período de 2 anos. Métodos: Análise retrospetiva de um subgrupo de 73 doentes que participaram num ensaio clínico randomizado e controlado, prospetivo, multicêntrico, longitudinal e intervencional, concebido para avaliar o efeito neuroprotetor da aplicação tópica de brimonidina e somatostatina numa coorte de diabéticos tipo 2 sem DR ou com DR ligeira [nível Early Treatment Diabetic Retinopathy Study Group (ETDRS) igual ou inferior a 35]. Os doentes foram seguidos por 2 anos ou até ao desenvolvimento de edema macular clinicamente significativo (CSME) ou retinopatia diabética proliferativa (PDR). A cada 6 meses, foram realizados exame oftalmológico completo e tomografia de coerência ótica (OCT). No início do estudo e aos 24 meses, foi realizada fotografia de fundo ocular (CFP). O subgrupo foi estudado como um todo por não terem sido encontradas diferenças significativas entre os doentes tratados e não tratados.Resultados: Foram considerados para análise os 73 doentes do estudo original seguidos na Associação para Investigação Biomédica em Luz e Imagem (AIBILI), Coimbra. Quatro olhos progrediram de edema macular subclínico (SCME) para CIME, enquanto 5 olhos progrediram de espessura retiniana normal (Normal-RT) para SCME. Estes 9 doentes apresentavam, no início do estudo, um aumento estatisticamente significativo da espessura dos anéis interno e externo da retina (p=0.029 e p=0.009). Ao longo dos 2 anos, ocorreu uma diminuição estatisticamente significativa nas espessuras do subcampo central na camada nuclear externa + segmento interno (ONL+IS) (p=0.004) e dos anéis interno e externo na camada de células ganglionares + plexiforme interna (GCL+IPL) (p=0.001 e p=0.002, respetivamente). No início do estudo, um maior número de doentes com nível ETDRS 35 (26.3%) apresentava CIME (p=0.012); esta relação manteve-se aos 24 meses, sendo que CIME estava presente em 33.3% dos doentes com nível ETDRS 35, comparativamente a 7.3% dos doentes com nível 10-20.Conclusões: Devido ao tamanho reduzido da amostra e ao curto período de follow-up, não foram encontrados marcadores surrogados de progressão para CIME. Contudo, uma espessura aumentada nos anéis interno e externo da retina, particularmente no quadrante temporal, poderá ser um potencial biomarcador de progressão da DR.
Objective: To characterize the initial stages of diabetic retinopathy (DR) and to identify, in a group of type 2 diabetic patients with no or mild DR, surrogate outcomes of progression to center-involving macular edema (CIME) in a 2-year follow-up period. Methods: Retrospective analysis of a subgroup of 73 patients that participated in a prospective, multicentric, longitudinal, interventional, randomised controlled clinical trial designed to evaluate the neuroprotective effect of topical brimonidine and somatostatine in a cohort of type 2 diabetic patients with no or mild DR [Early Treatment Diabetic Retinopathy Study Group (ETDRS) level less or equal to 35]. All patients were followed for 2 years or until the development of clinically significant macular edema (CSME) or proliferative DR (PDR). Ophthalmological examination and optical coherence tomography (OCT) were performed at each 6 months. Colour fundus photography (CFP) was performed at baseline and at 24 months. No statistically significant differences were found between treated and untreated groups thus the subgroup was studied as a whole. Results: The 73 patients from the original study followed in Association for Innovation and Biomedical Research on Light and Image (AIBILI), Coimbra, were considered for analysis. Four eyes progressed from subclinical macular edema (SCME) to CIME, whereas 5 eyes progressed from normal retinal thickness (Normal-RT) to SCME. These 9 patients presented with a statistically significant increased baseline thickness in both inner and outer rings (p=0.029 and p=0.009). In the 2-year period, there was a statistically significant decrease for the outer nuclear + inner segment layer (ONL+IS) thickness in the central subfield (p=0.004) and for the ganglion cell + inner plexiform layer (GCL+IPL) thickness in both the inner and the outer rings (p=0.001 and p=0.002, respectively). More patients (26.3%) with ETDRS level 35 presented with CIME at baseline (p=0.012), and this was also verified at 24 months, with 33.3% of the patients in ETDRS level 35 presenting CIME compared to 7.3% of patients in level 10-20. Conclusions: Due to the reduced size of the sample and the short follow-up period, surrogate outcomes of progression to CIME were not found. However, it seems that an increased thickness of both the inner and the outer rings of retina, particularly in the temporal quadrant, can be a potential biomarker of DR progression and development of CIME.
