dc.description.abstractIntrodução: A multimorbilidade é comum nos adultos portugueses e associa-se a piores consequências em saúde, porém a sua gestão adequada tem benefício para o doente ainda que exija carga de trabalho adicional para o médico assistente. A empatia é um elemento chave da relação médico-doente e a perceção positiva da sua presença associa-se a melhores consequências em saúde. Menores níveis de empatia foram encontrados em médicos com maiores taxas de esgotamento profissional. O presente estudo visa determinar se existe correlação entre o grau de sobrecarga dos médicos de família com a gestão de doentes com multimorbilidade e os níveis de empatia avaliados nos mesmos. Materiais e Métodos: O Questionário de Avaliação da Sobrecarga da Gestão da Multimorbilidade em Medicina Geral e Familiar (SoGeMM-MGF) foi desenvolvido por Filipe Prazeres, adotando o formato de um instrumento pré-existente. Foi aplicado a 32 médicos em 2 tempos, no seu local de trabalho, para verificação da confiabilidade.Realizou-se um estudo observacional e transversal, com aplicação do Questionário de Empatia para Médicos (JSPE-vP) e SoGeMM-MGF a um total de 131 médicos, durante setembro e outubro de 2018. O convite de participação foi enviado por correio eletrónico e processou-se por via informática o envio e recolha dos questionários. Os dados foram estudados por estatística descritiva e inferencial.Resultados: Relativamente ao estudo da fiabilidade do SoGeMM-MGF foi determinado um valor alfa de Cronbach superior a 0,8 em ambos tempos de aplicação. No estudo de campo foi obtida uma amostra de 131 médicos, sendo 70 do sexo feminino e registou-se uma idade mediana de 50 anos. O valor médio do SoGeMM-MGF foi de 79,08, tendo-se dividido a amostra em indivíduos “Mais Sobrecarregados” (>P50) e “Menos Sobrecarregados” (≤P50), correspondendo o primeiro grupo a 45% da amostra. Em relação ao JSPE-vP, obteve-se um valor médio de 75,66, sendo a amostra dividida em indivíduos “Mais empáticos” (>P50) e “Menos empáticos” (≤P50), correspondendo o segundo grupo a 53,4% da amostra. A correlação de Spearman calculada entre os resultados do SoGeMM-MGF e JSPE-vP (P=0,496; ρ=−0,060) indica uma correlação negativa, fraca e estatisticamente não significativa.Discussão: Os resultados do estudo de fiabilidade do SoGeMM-MGF permitiram concluir o processo de validação e fazer uso deste no estudo de campo. A pontuação média obtida no JSPE-vP é inferior ao relatado na literatura, dado que merece exploração adicional em investigações futuras. A ausência de correlação significativa entre o grau de sobrecarga dos médicos com a gestão de doentes com multimorbilidade e os níveis de empatia medidos nos mesmos poderá ter múltiplas explicações. Existe, no entanto, margem para trabalhar a empatia com intervenções formativas com potencial benefício para médicos e doentes.Conclusão: Os objetivos inicialmente propostos para este estudo foram cumpridos. Não foi encontrada correlação significativa entre as pontuações do SoGeMM-MGF e JSPE-vP, porém recomenda-se a realização de estudos futuros com amostras mais representativas da generalidade dos médicos de família Portugueses.por
dc.description.abstractBackground: Multimorbidity is a common condition in the adult Portuguese population and it is associated with worse clinical outcomes, however there is evidence that its proper management is beneficial to the patient, though it may require additional workload for the physician. Empathy is a key element of the doctor-patient relationship and positive perception of its presence is associated with better clinical outcomes. Higher rates of physician burnout were correlated with lower physician reported empathy. The goal of this study is to determine if a correlation exists between the degree of overburdening of general practitioners with the management of multimorbid patients and the level of self-reported empathy measured in these physicians.Materials and Methods: The scale: “Questionário de Avaliação da Sobrecarga da Gestão da Multimorbilidade em Medicina Geral e Familiar” (SoGeMM-MGF) was developed by Filipe Prazeres and it uses the format of a preexisting instrument. It was applied to 32 doctors in two different instances, in their work place, for verification of reliability. An observational, cross-sectional study was conducted. 131 doctors responded to the Portuguese version of the Jefferson Scale of Physician Empathy (JSPE-vP) and SoGeMM-MGF questionnaires, during September and October of 2018. An e-mail invitation was sent to the participants and filled-out questionnaires were retrieved using an online server. The data was analyzed using descriptive and inferential statistics. Results: Regarding the reliability of SoGeMM-MGF a Cronbach’s alpha value was determined, being higher than 0,8 for both applications of the questionnaire. The field study obtained a sample of 131 doctors. The median age of the subjects was 50 years and 70 were female. The mean SoGeMM-MGF score was 79,08, and the sample was divided into “More Overburdened” (>P50) and “Less Overburdened” (≤P50) individuals. The first group represented 45% of the sample. As for the JSPE-vP, a mean score of 75,66 was obtained, and the sample was divided into “More Empathic” (>P50) and “Less Empathic” (≤P50) individuals. The second group represented 53,4% of the sample. The Spearman correlation calculated between both the SoGeMM-MGF and JSPE-vP scores (P=0,496; ρ=−0,060) represents a negative, weak and statistically insignificant correlation.Discussion: The results of the analysis of reliability for the SoGeMM-MGF questionnaire allowed the use of this instrument in the field study. The mean score for JSPE-vP was lower than previously reported, which is deserving of additional investigation in future studies. The absence of significant correlation between the level of self-reported empathy measured in the doctors responsible for multimorbid patients and their respective overburdening with the management of these patients may have multiple explanations. In this context, it is important to mention that there is room to improve medical empathy via educational interventions with potential benefit to both doctors and patients. Conclusion: The initially proposed goals for this study were accomplished. No significant correlation was found between SoGeMM-MGF and JSPE-vP scores. Further studies should require a sample which better represents the average Portuguese general practitioner.eng
