dc.description.abstractINTRODUÇÃO: A urticária crónica espontânea (UCE) é uma patologia prevalente, caracterizada por lesões de urticária e/ou angioedema por um período superior a 6 semanas. Numa percentagem significativa a UCE é resistente aos anti-histamínicos H1 de 2ª geração, mesmo em altas doses. Nestes casos está indicada a utilização de Omalizumab e em 4ª linha ciclosporina. O omalizumab apresenta um bom perfil de eficácia e segurança, sendo a única opção de 3ª linha com autorização de introdução no mercado para UCE.OBJECTIVO:Avaliar a eficácia e segurança do omalizumab após tratamento com ciclosporina A na UCE e a associação entre os parâmetros clínicos e laboratoriais com os padrões de resposta.MÉTODOS:Estudo restrospectivo nos doentes da Consulta de Urticária do Serviço de dermatologia dos CHUC submetidos à terapêutica com ciclosporina A e, posteriormente, omalizumab entre Julho de 2013 e Dezembro de 2018. Para cada doente avaliámos as características demográficas, clínico-evolutivas da UCE, alterações laboratoriais, duração do tratamento, período de follow-up e reacções adversas. Caracterizámos a eficácia em função da escala de actividade da urticária durante 7 dias (UAS7). No tratamento de dados utilizou-se o Stata versão 25, utilizando o teste coeficiente Rho de Spearman e respetivo teste de significância, teste t de Student (grupos dependentes ou emparelhados), teste Wilcoxon, teste U de Mann-Whitney e teste Shapiro-Wilk (teste de normalidade).RESULTADOS:A amostra era constituída por 28 doentes, 25 do sexo feminino e 3 do sexo masculino, idade média 47.61anos ± 13.60 tratados com omalizumab com uma dose de 300mg/kg, 23 após tratamento prévio com ciclosporina. Em 67.9% (19) doentes houve uma boa resposta da urticária ao omalizumab obtendo-se valores de UAS7≤6 ou 0, em 25.0% (7) doentes uma resposta parcial com 6≤UAS7≤16 e em 7.1% (2) não houve resposta. A taxa de resposta é idêntica nos doentes com e sem medicação prévia com ciclosporina. Não se identificaram reacções adversas com o omalizumab, ao contrário do caso da ciclosporina.CONCLUSÃO:O omalizumab é um fármaco eficaz em mais de dois terços dos casos e tem um bom perfil de segurança em doentes com UCE moderada a grave, resistente a anti-histamínicos H1,sendo o fármaco a eleger tendo em conta os benefícios que aporta aos doentes e a segurança face à utilização “off-label” de ciclosporina A. Não encontrámos contudo, evidências de que a medicação anterior com ciclosporina condicione a eficácia do tratamento com omalizumab.por
dc.description.abstractINTRODUCTION: Chronic spontaneous urticaria (UCE) is a prevalent pathology characterized by urticarial lesions and / or angioedema for a period of more than 6 weeks. In a significant percentage of patients, UCE is resistant to second generation H1 antihistamines, even in high dosis. In these cases the use of montelukast, cyclosporin A and / or Omalizumab is indicated. The latter presents a good profile of efficiency and safety, being the only option of the 3rd line with marketing authorization for UCE. OBJECTIVE: To evaluate the efficiency and safety of Omalizumab after treatment with Ciclosporin A in the ECU and the association between clinical and laboratory parame-ters with response patterns. METHODS: Retrospective study in the Urticaria Consultation of the Department of Dermatology of CHUC, submitted to therapy with Cyclosporin A and later Omalizumab between July 2013 and December 2018. For each patient we evaluated the demographic, clinical and evolutionary characteristics of the UCE, laboratory and treatment duration, follow-up period, and adverse reactions to Cyclosporine. We characterized its efficiency according to the scale of activity of urticaria for 7 days (UAS7). For the data analysis was used the Stata version 25, using the test Spearman's Rho coefficient and respective significance test, Student's t test (dependent and paired groups), Wilcoxon test, Mann-Whitney U test and Shapiro-Wilk test (normality test). RESULTS: The sample consisted of 28 patients, 25 females and 3 males, average age 47.61 ± 13.60 years treated with Omalizumab at a dose of 300 mg/ kg, 23 after previous treatment with cyclosporin. In 67.9% (19) patients there was a good urticaria response to Omalizumab with UAS7≤6 or 0 values, 25.0% (7) patients had a partial response with 6 ≥UAS7≤16 and at 7.1% (2) there was no response. We have been able to affirm that there is no evidence that medication with Ciclosporin condition the effectiveness of treatment with Omalizumab. CONCLUSION: Omalizumab is an effective drug in more than two thirds and has a good safety profile in patients with moderate to severe UCE resistant to H1 antihista-mines and the off-label use of Ciclosporin A, the drug to be chosen taking into account the benefits it brings to patients.eng
