Abstract: INTRODUCTION: Paediatric Palliative Care (PPC) includes care of the sick child and the support to their family, being Europe where that this type of care is more developed and implemented. In 2018, Portugal became one of the first countries worldwide to legislate the foundation of intra-hospital support teams in PPC. Primary health care (PHC) is the first level of provision of health care, where the General Practitioner (GP) can intercede in the care of children with PPC needs. Therefore, GPs should be the first level of care for the child and family from the outset.OBJETIVES: The main goal of this research was to evaluate the clinical experience in PPC of a group of GPs working in health centers in the central region of Portugal. Secondary goal was the evaluation of the importance attributed by them to this area of health care delivery and to the training of these professionals in Palliative Care (PC). Finally, we sought to infer the interviewees’ opinion about the main barriers to the progress of PPC in Portugal.MATERIALS AND METHODS: Descriptive cross-sectional study. To achieve the objectives, a questionnaire was created consisting of sixteen questions of closed responses. It was submitted to a validation process, in order to increase the confidence of the instrument. Subsequently, the questionnaire was applied to a total of ninety-two GPs in Family Health Units and Primary Health Care Units. The questionnaire was submitted to descriptive and inferential statistics. For the purpose of carrying out the work, the necessary authorizations of the ethics committees were obtained.RESULTS: Although most respondents (94.6%) had contacted with adult PC, only 54.3% had clinical experience with PPC. The percentage of physicians who denied adequate and regular psychological support (44.6%) was higher than the percentage of respondents who admitted their existence (43.5%); the answers given by the female gender (46.9%), younger professionals (75,6%) and doctors with less years of experience (73.2%) showed the greatest lack of psychological support. Though more than 90% of the respondents considered that the psychological and economic support to parents/caregivers was important, 34.8% said they were told that this support did not exist. Almost all respondents (97.8%) considered PC important in clinical practice, which was reflected in the confirmation of previous training in 58.7% of the sample. The main barrier to the development and implementation of PPC programs in Portugal and to the teams was lack of training: 51.1% and 52,2%, respectively.DISCUSSION: It was found that clinical experience in CPP of most respondents is scarce. The main barrier was the lack of training, particularly felt by the oldest doctors and/or the ones with more time of clinical experience. Female doctors, the youngest doctors and the ones with shorter careers referred the greater need of psychological support as well as the lack of its supply. Economic and psychological support has proved to be insufficient, although most respondents considered it to be of utmost importance. Contrary to our expectations, hospital care was preferred rather than home care, which may be due the lack of support felt by the families; if such support existed, it would help to care their children at home. Most of the respondents, particularly the male gender, considered PC important in their clinical practice, giving relevance to the need for training in this area.CONCLUSION: It is necessary to implement a national model based on multiprofessional training of mandatory attendance. This model should privilege both the economic and psychological support of parents/caregivers and home care. It also should benefit PHC as a link between the sick child and his family in hospital care.
INTRODUÇÃO: Os Cuidados Paliativos Pediátricos (CPP) abrangem não só o cuidado à criança doente como também apoiam a sua família, sendo a Europa o local onde esses cuidados se encontram mais desenvolvidos. Em 2018, Portugal tornou-se um dos primeiros países do mundo a legislar quanto à instituição das equipas intra-hospitalares de suporte em CPP. Mas, sendo os Cuidados de Saúde Primários (CSP) o primeiro nível de prestação de cuidados de saúde, onde o médico de Medicina Geral e Familiar (MGF) está habilitado a intervir nos cuidados da criança, deverá ser o primeiro nível de prestação destes cuidados, à criança e família, desde o início.OBJETIVOS: Esta investigação teve como objetivo principal avaliar a experiência clínica em CPP, de um grupo de médicos de MGF, em exercício de funções em centros de saúde na região centro de Portugal. Como objetivo secundário, avaliou-se a importância atribuída pelos mesmos a esta área de prestação de cuidados de saúde e à formação destes profissionais em Cuidados Paliativos (CP). Por último, procurou-se inferir a opinião dos inquiridos acerca das principais barreiras ao progresso dos CPP em Portugal.MATERIAIS E MÉTODOS: Estudo descritivo transversal. Tendo em conta a concretização dos objetivos, foi elaborado um questionário constituído por dezasseis questões de resposta fechada. Este foi submetido a um processo de validação, com o intuito de aumentar a confiança do instrumento. Posteriormente, foi aplicado a Unidades de Saúde Familiar e Unidades de Cuidados de Saúde Primários a um total de noventa e dois médicos de MGF. Os questionários foram submetidos a estatística descritiva e inferencial. Para efeitos de execução do trabalho, foram obtidas as devidas autorizações das comissões de ética.RESULTADOS: Ainda que a quase totalidade de inquiridos (94,6%) tivesse contactado com os CP de adultos, somente 54,3% teve experiência clínica com os CPP. A percentagem dos médicos que negou apoio psicológico adequado e regular (44,6%) foi superior à que admitiu a sua existência (43,5%), sendo o género feminino (46,9%), os profissionais mais jovens (75,6%) e os com menos anos de experiência clínica (73,2%) os que mais demonstraram a carência de acompanhamento psicológico. Embora mais de 90% dos inquiridos tivesse considerado importante o apoio psicológico e económico aos pais/cuidadores, 34,8% da amostra referiu que lhes foi relatado a inexistência dos mesmos. A quase totalidade dos respondentes (97,8%) considerou os CP importantes na prática clínica, o que se refletiu na confirmação de formação prévia em 58,7% da amostra. A principal barreira quer ao desenvolvimento e implementação de programas de CPP em Portugal quer para as equipas foi a falta de formação: 51,1% e 52,2%, respetivamente.DISCUSSÃO: Verificou-se que a experiência clínica em CPP da maioria dos inquiridos, médicos de MGF, é escassa. A principal barreira identificada foi a falta de formação, sentida particularmente pelos mais velhos e/ou com mais anos de experiência clínica. Quanto à necessidade de apoio psicológico, as médicas, os mais jovens e os com carreiras mais curtas foram os que referiram maior necessidade, e falta de oferta, de apoio psicológico adequado. Assistência económica e psicológica aos pais/cuidadores demonstrou ser insuficiente, apesar de reconhecidamente muito importante pela maioria dos inquiridos. Quanto à preferência pelo cuidado hospitalar versus cuidado no domicílio, as respostas obtidas denotaram a preferência pelos cuidados hospitalares, contrariamente ao esperado, o que pode ser devido à falta de apoio sentido pelas famílias, que permitisse cuidar das suas crianças em casa. A maioria dos inquiridos, particularmente o género masculino, considerou os CP importantes na sua prática clínica, dando relevância à necessidade de formação nesta área. CONCLUSÃO: É necessária a implementação de um modelo nacional que se baseie na formação multiprofissional de presença obrigatória, que privilegie tanto o apoio económico e psicológico dos pais/cuidadores, como o cuidado domiciliário e privilegie os CSP como elo entre a criança doente e a sua família aos cuidados hospitalares.
