Abstract: Introduction: Although the distinction between normal and pathological anxiety is particularly difficult in the perinatal period; anxiety disorders affect approximately 10% of women in this period. Early detection is imperative because of the anxiety implications in the perinatal period. To our knowledge, there is only one screening instrument, ERAP, which evaluates four dimensions: 1 - Excessive Worry/Specific fears; 2 - Perfectionism/ Control/ Trauma; 3 - Social Anxiety; 4 - Acute anxiety and adjustment. Objective: To test the factorial structure of ERAP and to analyze its psychometric properties, namely construct validity, convergent-divergent validity, as well as reliability (internal consistency and temporal stability). Methods: 606 pregnant women, between 10 and 31 weeks of gestation (mean weeks of gestation=17.07±4.813) answered a set of self-reported questionnaires containing ERAP, ERDP-24 and POMS. From this sample, 251 participants also answered to a diagnostic interview. In the postpartum, around the third/fourth month (mean age of the baby in weeks=14.14±4.457), they answered again to ERAP, in order to analyze the temporal stability of the scale. Results: The factorial structure of the initial model revealed poor adjustment indices. After the elimination of the items with loadings lower than .40 (1 and 2) and correlated five pairs of errors, the second order model revealed an acceptable fit (X²/gl=3.9263; TLI=.8855; CFI=.8967; GFI=.8469; RMSEA=.0695). The four dimensions of ERAP-29 demonstrated good internal consistency, with Cronbach alphas between .837 (social anxiety) and .890 (acute anxiety and adjustment). In the study of convergent-divergent validity, significant moderate to high correlations between the dimensions of ERAP, PDSS and POMS were found. The Mann Whitney U test showed excellent criterion validity, by showing that affected women presented significantly higher total and dimensional mean scores in ERAP-29. ROC analysis evidenced that the cut-off point that potentiated the Yoden Index (.7705) was of 28, associated with a sensitivity of 84.62% and a specificity of 92.44%. Discussion and Conclusion: Globally, ERAP-29 revealed good construct validity (acceptable fit), convergent-divergent validity and reliability, as well as excellent accuracy in screening for anxiety disorders. This scale fills the gap with regard to the lack of screening instrument for anxiety in pregnancy, and it is ready to be used for this purpose.
Introdução: Embora a distinção entre ansiedade normal e patológica seja particularmente difícil no período perinatal, as perturbações de ansiedade afetam aproximadamente 10% das mulheres neste período. A deteção precoce é um imperativo devido às implicações da ansiedade no período perinatal. Até agora e, tanto quanto seja do nosso conhecimento, existe apenas um instrumento de rastreio, a Escala de Rastreio da Ansiedade Perinatal (ERAP) com quatro dimensões: 1 - Preocupação excessiva/Medos específicos; 2 - Perfeccionismo/ Controlo/Trauma; 3 - Ansiedade Social; 4 - Ansiedade aguda e ajustamento.Objetivo: Testar a estrutura fatorial da ERAP e analisar as suas propriedades psicométricas, nomeadamente validade de constructo, concorrente e convergente-divergente, bem como a fidelidade (consistência interna e estabilidade temporal).Métodos: 606 grávidas, entre 10ª e a 31ª semanas de gestação (Média de semanas de gestação=17.07±4.813) responderam a questionários de autopreenchimento que continham a ERAP, a ERDP-24 e a POMS. Desta amostra 251 participantes foram submetidas a uma entrevista diagnóstica. No pós-parto, aproximadamente no terceiro/quarto mês (Idade média do bebé em semanas=14.14±4.457) responderam novamente à ERAP de forma a avaliar a estabilidade temporal da escala.Resultados: A estrutura fatorial do modelo inicial revelou maus índices de ajustamento. Após a eliminação dos itens com loadings inferiores a .40 (1 e 2) e correlacionados cinco pares de erros, o modelo de segunda ordem revelou um ajustamento aceitável (X²/gl=3.9263; TLI=.8855; CFI=.8967; GFI=.8469; RMSEA=.0695). As quatro dimensões da ERAP-29 demonstraram consistência interna boa, com alfas de Cronbach entre .837 (Ansiedade social) e .890 (Ansiedade aguda e ajustamento). No estudo da validade convergente-divergente, obteve-se correlações significativas entre as dimensões da ERAP e a PDSS; da POMS a única dimensão que não se correlacionou com a ERAP foi o Vigor. O teste U de Mann Whitney evidenciou excelente validade de critério, ao relevar que as mulheres com perturbações de ansiedade apresentaram pontuações totais e dimensionais significativamente superiores na ERAP.29. Através da análise de curvas ROC, verificou-se que o ponto de corte que potenciou o Yoden Index (.7705) foi o 28, associado a uma sensibilidade de 84.62% e uma especificidade de 92.44%.8Discussão e Conclusão: A presente adaptação da ERAP, globalmente, revelou boa validade de construto (ajustamento aceitável), convergente-divergente e fidelidade, bem como excelente acuidade para o rastreio de perturbações de ansiedade. Vem colmatar a falta de um instrumento de rastreio da ansiedade na gravidez, podendo começar a ser utilizada para esse fim.
