Abstract: Os tumores da hipófise são as terceiras neoplasias intracranianas mais frequentes. Devido ao aumento da longevidade, doenças, anteriormente não associadas aos idosos, começam a ser diagnosticadas nessa população, até agora pouco estudada.Foi efetuado um estudo retrospetivo, que incluiu 21 doentes com tumor da hipófise, com idade ao diagnóstico igual ou superior a 80 anos. O processo de recolha de dados consistiu na revisão dos processos clínicos e exames radiológicos dos participantes. Os dados recolhidos foram inseridos numa base de dados, processados informaticamente e analisados estatisticamente. Foi considerado estatisticamente significativo um valor de p<0,05.Dos 21 doentes, 13 eram mulheres e 8 homens, com idade média ao diagnóstico de 83,4±2,2 anos, com idade máxima de 88 anos. O diagnóstico foi incidental em 61,9% dos doentes. 81% dos doentes já apresentava sintomas neurológicos (64,7%), visuais (53,3%) e/ou endocrinológicos (17,6%). Relativamente ao doseamento da prolactina, encontrou-se aumentada em 47,6% dos casos, com valores diagnósticos de prolactinoma em 3 doentes. A IGF-1 encontrava-se aumentada num doente, com clínica compatível de acromegalia. 71,4% dos doentes apresentava algum grau de insuficiência hipofisária, mais preponderante nas linhagens gonadotrópica e somatotrópica. Encontrou-se hiponatrémia em 15,8% dos doentes. Dos doentes que realizaram avaliação imunohistoquímica, detetaram-se 3 adenomas gonadotrofos e 1 lactotrofo.Relativamente às dimensões tumorais, predominaram macroadenomas (91%), dos quais cerca de 95% apresentavam extensão/invasão extrasselar. Constatou-se haver uma correlação entre a dimensão dos tumores e o número de eixos hipofisários afetados. Quanto à terapêutica, predominou o tratamento médico (71,4%), quer como tratamento dirigido aos tumores (prolactinomas/acromegalia: normalização da prolactina/IGF-1), como para correção das insuficiências hipofisárias e hiperprolactinemia. 3 doentes realizaram cirurgia e registou-se redução parcial/subtotal do volume tumoral, revelando-se esta opção terapêutica como mais significativa na redução tumoral comprovada imagiologicamente, comparada com o tratamento médico. O tratamento conservador foi escolhido em 3 doentes.A frequência de diagnósticos incidentais já com clínica atribuível aos adenomas hipofisários pode refletir uma possível desvalorização das manifestações encontradas nos doentes idosos, por dificuldade no diagnóstico diferencial com o envelhecimento e condições crónicas comuns nesta faixa etária, levando a um diagnóstico tardio desta patologia. Daí ocorre uma diminuição na qualidade de vida, quer pelo impacto neuro-oftalmológico como pelas insuficiências hormonais, possivelmente revertível ou tratável. O tratamento deve ser definido não só em função do tumor, mas também individualizado perante as particularidades de cada doente.
Pituitary tumours are the third most common intracranial neoplasms. Due to the increasing life-expectancy, diseases, previously not associated with the elderly, are starting to be diagnosed in this population, thus remaining so far understudied.To study the pathology of pituitary tumours among the elderly, a retrospective study was conducted, including 21 patients with a pituitary tumour diagnosed at an age equal to or greater than 80 years. The data collection process consisted of reviewing the clinical files and radiological examinations of the participants.The collected data was entered into a database, processed and statistically analysed, with a value of p<0.05 being considered statistically significant.Out of these 21 patients, 13 were female and 8 male, with mean age of 83.4±2.2 years, and a maximum age of 88 years at diagnosis. The diagnosis was incidental in 61.9% of patients. 81% of patients already had neurological (64.7%), visual (53.3%) and / or endocrinological symptoms (17.6%) at diagnosis. As for the prolactin levels, they were found to be increased in 47.6% of the cases, with 3 patients reaching the levels to be diagnosed as a prolactinoma. IGF-1 was increased in one patient, accompanied by a clinically compatible acromegaly. 71.4% of the patients presented some degree of pituitary insufficiency, more prevalent in the gonadotrope and somatotrope lines. Hyponatremia was found in 15.8% of patients. In the patients that underwent immunohistochemical evaluation, 3 gonadotropic adenomas and 1 lactotroph adenoma were detected.Regarding the tumor dimensions, macroadenomas predominated (91%), about 95% of which presented extrassellar extension / invasion. A correlation between the size of the tumors and the number of pituitary axes affected was found. Regarding treatment options, medical therapeutic (71.4%) was predominant, either as a treatment for tumors (prolactinomas/acromegaly: normalization of prolactin/IGF-1 levels) or for the correction of pituitary insufficiencies and hyperprolactinemia.3 patients underwent surgery by partial / subtotal tumorectomy, proving this therapeutic option to be more effective in tumour reduction compared to medical treatment. A conservative approach was chosen in the case of 3 patients.The frequency of incidental diagnoses in already symptomatic patients, with manifestations attributable to pituitary adenomas may reflect a possible devaluation of these symptoms found in elderly patients, as a differential diagnosis of aging and common chronic conditions in this age group, leading to a late diagnosis of this neoplasm. This results in a diminished quality of life, caused by both the neuro-ophthalmological and the hormonal insufficiencies, both being possibly revertible or treatable. Finally, considering the treatment, it should not only be chosen according the characteristics of the tumor, but should be individualized for each patient and specific particularities.
