Abstract: Introdução: A Medicina Centrada na Pessoa (MCP) é um método que avalia o doente comoum todo. Este método está divido em 4 componentes principais (Explorando a Saúde, adoença e a Experiência de doença; Entendendo a Pessoa como um Todo; Elaborando umPlano Conjunto de gestão de problemas; Intensificação da relação entre a Pessoa e omédico). Anualmente são publicados Indicadores dos Cuidados de Saúde Primários compilados pela Administração Central do Sistema de Saúde (ACSS) que permitem uma aferição de diversos componentes da prática médica diária dos Médicos de Medicina Geral e Familiar.Objetivo: Avaliar a relação entre a autopercepção médica de praticar o método centrada na pessoa (MCP) e como tal se relaciona com os indicadores de saúde da ACSS.Tipo de Estudo: Observacional.Métodos: O questionário de MCP foi enviado a 482 médicos de Medicina Geral e Familiarque trabalham quer em Unidade de Saúde Familiar (USF) A ou B ou Unidade de Cuidadosde Saúde Personalizados (UCSP) da ARS-Centro, no ACeS Baixo Mondego. Solicitaram-seos indicadores individuais de saúde a que, os médicos não acederam no seu fornecimento,assim a autora utilizou os indicadores de cada tipo de unidade do ACeS, dados estesfornecidos pela própria unidade. Realizou-se estatística descritiva e inferencial não paramétrica pelos testes U de Mann-Whitney e Kruskall-Wallys dos dados obtidos.Resultados: A amostra é constituída na sua maioria por médicas (75,9%). Nas respostas aoquestionário sobre MCP percebeu-se que apenas a média dos capítulos e MCP total tinhadistribuição normal. Por tipo de unidade percebem-se diferenças significativas para a médiado Capítulo 2 e os indicadores medidos. A comparação entre a pontuação global deautopercepção de praticar MCP e os vários indicadores medidos não mostra diferençasestatisticamente significativas. Tendencialmente as unidades USF obtiveram melhores resultados no Capítulo 2 – “Entendendo a Pessoa como um todo” (MCP) e melhores resultados nos indicadores, exceto no indicador “número total de consultas”.Discussão: Nesta amostra não houve diferenças significativas nos dados em função do sexo. Ademais, verificou-se uma relação entre os indicadores de saúde e a autopercepçãode realizar MCP, sendo que, as unidades USF, que têm maior média no Capítulo 2 –“Entendendo a Pessoa como um todo”, apresentaram melhores resultados na generalidadedos indicadores, o que pode ser explicado pela forma de funcionamento destas unidades.Para além disso, não se conseguiram inferir diferenças entre a pontuação global doquestionário MCP e os vários indicadores medidos. Por último, verificou-se, em relação aos indicadores de saúde e a medicina praticada em cada modelo organizativo, que a UCSP tem na generalidade piores indicadores de saúde, mas maior “número de consultas”, o pode ser devido à forma de organização da unidade.Conclusão: Na amostra em estudo, os médicos da unidade USF percecionam maiorrealização de MCP (Capítulo 2 – “Entender a pessoa como um todo”) e apresentamgenericamente melhores indicadores de Saúde. A UCSP apesar de apresentar pioresindicadores de saúde, é a unidade que realiza “maior número de consultas”.
Background: The Patient-Centred Medicine (PCM) is a method that evaluates the person asa whole. This method is divided in 4 main components (Exploring health, disease and illnessexperience; Understanding the person as a whole; Developing a problem management plantogether; Enhancing the doctor -patient relationship). Annually, the endpoints are published by the Administração Central dos Sistemas de Saúde (ACCS), data from Portuguese healthMinistry, that gather a health assessment of the General Practitioners (GP).Objectives: To evaluate the relationship between the medical self-perception of practicing the PCM and the healthcare endpoints provided by ACSS.Design: Observational.Methods: The PCM questionnaire was sent to 482 family/general doctors, that belong to 3different types of primary care unities (USF-A, USF-B e UCSP) from ACeS Baixo Mondego.Individual health endpoints were requested, which the doctors did not access in their supply,so the author used the endpoints of each type of primary care unity of the ACeS, given bythe unit itself. Descriptive and non-parametric inferential statistics were performed by theMann-Whitney and Kruskall-Wallys U tests of the obtained data.Results: The sample consisted mostly of female doctors (75.9%). In the responses to thePCM questionnaire it was noticed that only the mean of the chapters and total PCM hadnormal distribution. By type of unit significant differences are observed for the mean ofChapter 2 and the measured indicators. The comparison between the overall self-perceptionscore of practicing PCM and the various measured indicators does not show statisticallysignificant differences. USF units tended to perform better in Chapter 2 – "Understanding the person as a whole" and had better results on endpoints, except for the "total number of consultations”.Discussion: In this sample, there were no statistically significant differences in the dataaccording to sex. In addition, there was a relationship between health indicators and the selfperceptionof PCM, with the USF units with the highest mean in Chapter 2 –"Understanding the person as a whole" presented better results in most of the indicators.That can be explained by the organization of these units. Furthermore, it was not possible toinfer differences between the overall PCM questionnaire score and the various measuredindicators. Finally, in relation to the health endpoints and the medicine practiced in each organizational model, UCSP has generally worse health indicators, but a greater "number of consultations" can be due to the organization of the unit.Conclusions: In the sample, physicians at the USF unit perceive greater achievement ofPCM (Chapter 2 – "Understanding the person as a whole") generally present better healthindicators. The UCSP, despite presenting worse health indicators, is the unit that performs"Greater number of consultations".
