dc.description.abstract | Introdução: Diversos autores têm reportado que a prevalência da infeção por Helicobacter pylori em doentes com úlcera péptica complicada por hemorragia é inferior à que se regista nos doentes com úlcera péptica não complicada. O presente estudo pretende determinar a verdadeira prevalência da infeção em doentes com úlceras pépticas sangrantes, recorrendo a métodos invasivos e não invasivos.Métodos: Sessenta e quatro doentes com hemorragia digestiva alta e úlcera péptica ativa confirmada por endoscopia foram integrados no estudo. Os indivíduos que já tivessem sido submetidos a erradicação prévia de Helicobacter pylori não foram considerados para inclusão. As características clínicas da amostra foram colhidas, prospectivamente, entre julho de 2017 e junho de 2018. A infeção por Helicobacter pylori foi determinada através de estudo histológico (biopsias do corpo e antro), teste respiratório com ureia marcada por 13C e testes serológicos. Os doentes foram considerados Helicobacter pylori negativo quando obtivessem resultados negativos em todos os métodos de diagnóstico, e considerados Helicobacter pylori positivo se apresentassem pelo menos um resultado positivo.Resultados: A taxa de prevalência determinada neste grupo de doentes foi de 84,4%. Esta prevalência aumentou para 90,3% quando se excluíram os doentes que estavam medicados com anti-inflamatórios não esteroides. O consumo destes fármacos foi mais frequente no grupo Helicobacter pylori negativo (70%) que no grupo Helicobacter pylori positivo (48,1%) (p=0,305). A proporção de casos sem infeção por Helicobacter pylori e sem consumo de anti-inflamatórios não esteroides foi muito baixa (4,7%). Não encontrámos diferenças nas características clínicas e no prognóstico entre doentes infetados e não infetados.Conclusão: A prevalência da infeção por Helicobacter pylori nos doentes com úlcera péptica complicada por hemorragia é muito elevada, ao contrário do estabelecido previamente por outros autores. Desta forma, deverá ser realizada uma análise de custo-eficácia para a realidade portuguesa, porque a erradicação empírica poderá estar indicada neste grupo de doentes. | por |
dc.description.abstract | Background: Several authors have reported that the prevalence of Helicobacter pylori infection in patients with bleeding peptic ulcer is lower than the prevalence in patients with non-complicated ulcers. The present study aims to determine the true prevalence of the infection in bleeding peptic ulcer, with both invasive and non-invasive methods.Methods: Sixty-four patients with upper gastrointestinal bleeding and an endoscopically verified active peptic ulcer were enrolled in the study. Individuals who had already been submitted to prior eradication of Helicobacter pylori were not considered for inclusion. Clinical data of all subjects were prospectively collected between July 2017 and June 2018. Helicobacter pylori infection was assessed by histologic examination (corpus and antral biopsies), a 13C-urea breath test and serologic examination. Patients were considered Helicobacter pylori negative if all tests were negative and considered Helicobacter pylori positive if at least one test was positive.Results: The prevalence rate determined in this group of patients was 84.4%. This prevalence increased to 90.3% when patients taking nonsteroidal anti-inflammatory drugs were excluded. The use of these drugs was more frequent in Helicobacter pylori negative patients (70%) than in Helicobacter pylori positive patients (48.1%) (p=0.305). The proportion of cases without Helicobacter pylori infection and without nonsteroidal anti-inflammatory drugs intake was very low (4.7%). We found no differences in clinical features and prognosis between infected and non-infected patients.Conclusion: The prevalence of the Helicobacter pylori infection in patients with bleeding peptic ulcer is very high, unlike the previous reports from other authors. Therefore, a national cost-effectiveness analysis should be performed because empirical eradication could be indicated in this group of patients. | eng |
