Abstract: Introdução:A empatia médica é contributo essencial para relação medico-doente, sendo fator relevante e constitutivo da vertente humana e profissional da Medicina.Da perspetiva de quem consulta, a empatia que o doente sente do médico durante os seus contactos com este, pode ser avaliada, dentre outrsas, por duas escalas: CARE (The Consultation and Relational Empathy) e JSPPPE (Jefferson Scale of Patient Perceptions of Physician Empathy).Objetivos:Efetuar a adaptação cultural da CARE e realizar a sua validação populacional para o português concorrentemente com a JSPPPE em função dos seus valores absolutos e saber qual a escala com maior agradabilidade pelo respondente.Métodos:Foi feita a tradução, verificação da adaptação linguística e tradução de novo para Inglês da CARE, seguida de teste e re-teste, após o que foi feita aplicação populacional das escalas CARE e JSPPPE numa amostra de conveniência, em dias aleatórios, em local não acessível aos médicos, segundo o protocolo, após convite e assinatura de termo de consentimento informado. Realizou-se estatística descritiva e inferencial não-paramétrica com nível de significância para p<0,03.Resultados:Verificou-se confiabilidade pelo α de Cronbach de 0,982 e fiabilidade (ρ=0,982, p<0,001) na fase de adaptação cultural após a tradução para português e retro-tradução a inglês. Numa amostra de 200 respostas 119 (59,5%) de mulheres, 95 (47,5%) com idade >35 anos e 35 (17,5%) com idade ≥65 anos verificou-se correlação entre os valores absolutos das escalas, de ρ=0,764, p<0,001.Verificou-se maior agrado pela escala CARE para 137 (68,5%) dos respondentes, significativamente diferente entre sexos, a mulher mais agradada com CARE (65,0%) e o homem com JSPPPE (52,4%) (p=0,015) e sem diferenças por grupo etário (p=0,747) e por tempo de seguimento pelo médico (p=0,571). Não há diferença no valor absoluto de cada escala por sexos, CARE p=0,740 e JSPPPE p=0,850.Discussão e conclusões:Após o processo de adaptação cultural e de validação populacional concorrente verificou-se excelente correlação entre JSPPPE e CARE, tendo maior agradabilidade para os respondentes a CARE, mesmo que mais longa.Os presentes resultados permitem perceber não haver diferenças nos resultados entre ambas as escalas, mesmo que CARE permita obter conhecimentos sobre mais áreas que a JSPPPE.
Introduction:Medical empathy is an essential part in the doctor-patient relationship, so being a relevant factor for human and professional aspects in Medicine. From the patient's perspective, medical empathy can be measured among others by the scales JSPPPE (Jefferson Scale of Patient Perceptions of Physician Empathy) and CARE (The Consultation and Relational Empathy).Objectives:To carry out the cross-cultural adaptation of CARE and its populational concurrent validation to European Portuguese language with the JSPPPE, according to their absolute values and to know which scale is most agreeable to the respondent.Methods:Translation, verification of linguistic adaptation and back translation to English of CARE were made, followed by test re-test. Then, a populational application of the CARE and JSPPPE scales was done in a convenience sample, on random chosen days, in a non-accessible place to working physicians, according to ethically approved protocol, after invitation and signing in of an informed consent form. Descriptive and non-parametric inferential statistics were performed with significance level for p<0,03.Results:In cross-cultural adaptation and after translation to Portuguese and back translation and debriefing, an internal consistency by Cronbach’s alfa of 0,982 was found and a very good correlation was achieved in the test-retest operation (ρ=0,982, p<0,001).In a sample of 200 responses, of which 119 (59.5%) women, 95 (47.5%) <35 years old, 35 (17.5%) ≥ 65 years old, 106 (53.0 %) had the same doctor for 3 years, excellent correlation between the absolute values of the scales of ρ=0.764, p<0.001 was verified.The CARE scale was most appreciated by 137 (68.5%) respondents, significantly different between genders, women with CARE (65.0%) and man with JSPPPE (52.4%) (p=0.015) and without differences by age group (p=0.747) and by follow-up by the physician (p=0.571). There is no difference in the absolute value of each scale between sexes, CARE p=0.740 and JSPPPE p=0.850.Discussion and conclusions:An excellent correlation between JSPPPE e CARE was found. So CARE can now be used in Portugal to measure medical empathy, even if it is longer than JSPPPE. The present results show that there are no differences in results between the two scales, although CARE allows to obtain knowledge on more areas than the JSPPPE.
