Abstract: Bipolar disease is characterized by a transition between manic and depression states. Lithium is specified for the treatment of this disease and its benefits have been reported since the XIX century. It is the only treatment that shows a clear pattern in the reduction of the risk of suicide in bipolar disorder. The associated mechanisms are not fully understood, however, it is known that it has important functions in neuroplasticity, neuronal survival, remyelination, regulation of neurotransmitters and resynchronization of circadian rhythm. Lithium is absorbed in the upper gastrointestinal tract. It has a bioavailability of approximately 100% and does not bind to plasma proteins. Additionally, lithium does not undergo hepatic metabolization, its half-life time is approximately one day and it is excreted by renal route. The maintenance dose is 0.4 to 1.0 mmol/L, however it should be adapted to the patient's age and physiological condition. Therapeutic monitoring of lithium plasma levels is recommended to assess the efficacy of therapeutic response and to prevent toxicity. The relationship between bipolar disease and lithium has been analysed in recent and important scientific studies. It was found that lithium increases the integrity of white and gray matter in bipolar disease, reverses volume losses of the hypothalamus and thalamus, and reduces dementia and estimated brain age. Currently, new therapeutic perspectives are being investigated in neurodegenerative diseases such as Alzheimer's disease, Parkinson's disease and Huntington's disease. Clinical studies have found that lithium prevents the aggregation of α-sinuclein, improves cognitive and motor function and shows antioxidant, neuroprotective and anti-inflammatory effects.This paper work addresses updates on lithium therapy in bipolar disorder and some of the latest scientific studies on its important neurobiological effects regarding to treatment and prevention of some diseases.
A doença bipolar caracteriza-se por uma alternância entre estados de mania e depressão. O lítio está indicado para o tratamento desta doença e os seus benefícios são relatados desde o século XIX. É o único tratamento que demonstra reduzir o risco de suicídio na doença bipolar. Os seus mecanismos de ação não são totalmente conhecidos, no entanto, sabe-se que tem funções importantes na neuroplasticidade, sobrevivência neuronal, remielinização, regulação de alguns neurotransmissores e ressincronização do ritmo circadiano.O lítio é absorvido no trato gastrointestinal superior, apresenta uma biodisponibilidade aproximadamente de 100%, não se liga às proteínas plasmáticas, não tem metabolização hepática, o tempo de semi-vida é de aproximadamente um dia e é excretado por via renal. A dose de manutenção é de 0,4 a 1,0 mmol/L, porém deve ser adaptada à idade e condição fisiológica do doente. É recomendado efetuar monitorização terapêutica dos níveis plasmáticos de lítio para avaliar a eficácia da resposta terapêutica e prevenir toxicidade.A relação entre a doença bipolar e o lítio foi abordada em alguns estudos científicos recentes e importantes. Verificou-se que o lítio aumenta a integridade da substância branca e cinzenta na doença bipolar, consegue reverter as perdas de volume do hipotálamo e tálamo, e reduz a demência e a idade cerebral estimada.Atualmente, estão a ser investigadas novas perspetivas terapêuticas nas doenças neurodegenerativas como a doença de Alzheimer, doença de Parkinson e doença de Huntington. Estudos clínicos verificaram que o lítio impede a agregação das α-sinucleína, melhora a função cognitiva e motora e apresenta efeitos anti-oxidantes, neuroprotetores e anti-inflamatórios.Esta monografia aborda atualizações da terapêutica do lítio na doença bipolar e alguns dos mais recentes estudos científicos sobre os seus efeitos neurobiológicos importantes para o tratamento e prevenção de algumas doenças.
