Abstract: A Doença Inflamatória Intestinal (DII) compreende duas formas possíveis: a Doença de Crohn e a Colite Ulcerosa, que afectam a mucosa intestinal de forma distinta. A DII representa um problema de saúde mundial que se encontra globalmente em mudança. A etiologia da doença permanece por esclarecer, no entanto têm sido realizados grandes avanços na compreensão da patogénese da DII. Diversos factores podem estar na origem da doença entre os quais a susceptibilidade genética, a barreira intestinal e o meio ambiente. Presentemente estão identificados 163 loci de genes associados à DII, que afectam a resposta imunológica nos seus diferentes mecanismos. A alteração da barreira intestinal proporciona uma resposta imunológica anormal à microbiota intestinal, que também está alterada na doença. O ambiente envolvente dos doentes com DII apresenta, igualmente, grande impacto no desenvolvimento da patologia. Embora tenham ocorrido avanços importantes no conhecimento da patogénese da DII, estes têm-se revelado insuficientes e, futuramente, são necessários mais estudos para esclarecer os diversos mecanismos implicados na doença.
Crohn’s Disease and Ulcerative Colitis are two main types of Inflammatory Bowel Disease (IBD), which affects the intestinal mucosa in different ways. The IBD represents a world health problem that is changing globally. The etiology of this disease still remains unclear, although huge advances have been made in the comprehension of the pathogenesis of IBD. Many factors can be in the origin of this disease, among which the genetic susceptibility represents a main factor. Currently, there are 163 loci of genes associated to IBD that affects the immunologic response in its different mechanisms. The modification in the intestinal barrier leads to an abnormal immunologic response to the intestinal microbiotic, which is also changed in the disease. The environment in which the patients with IBD are also represents a huge factor in the development of this pathology. Although important advances had occur in the knowledge of the pathogenesis of IBD, they are insufficient and, in the future, it will be necessary more studies to clarify the different mechanism involved in the disease.
