Abstract: Com o avançar da idade, a população idosa fica cada vez mais suscetível a fraturas ósseas, graças à perda de massa óssea que advém de patologias normalmente associadas a esta faixa etária. As fraturas do fémur proximal, em particular as fraturas trocantéricas, apresentam-se como as mais recorrentes, sendo que as fraturas trocantéricas com obliquidade reversa constituem um maior desafio para o cirurgião ortopédico. Neste contexto, o principal método de tratamento recorre a sistemas de fixação óssea interna. No entanto, o facto de os sistemas existentes atualmente no mercado não se demonstrarem 100% eficazes, com a necessidade da sua substituição durante a vida do paciente, leva ao desenvolvimento de novas soluções.Neste seguimento, esta dissertação tem como principal objetivo a comparação do comportamento biomecânico de uma nova placa de contenção/compressão trocantérica (Trochanteric Plate of Contention/Compression -TPC) com uma placa extramedular de parafuso dinâmico (Dynamic Hip Screw – DHS), utilizada em larga escala no contexto referido. A avaliação numérica das placas incidiu sobre diferentes fases do processo de regeneração óssea de uma fratura trocantérica com obliquidade reversa: imediatamente após a inserção dos sistemas (I), recuperação parcial da fratura (II) e recuperação total da lesão (III). Neste estudo foi ainda analisada a influência dos músculos abdutores nas condições de carregamento dos conjuntos.As simulações numéricas realizadas ao longo deste trabalho foram feitas recorrendo ao software de cálculo por elementos finitos, ADINA®, tendo por base conjuntos constituídos por um modelo digital do fémur, da marca Sawbones® (ficheiro CAD #3403), bem como por modelos 3D das placas, TPC e DHS, previamente desenvolvidos através de sistemas de modelação CAD e CAE.A partir da análise dos resultados obtidos para a magnitude dos deslocamentos, assim como para a distribuição das tensões de von Mises e das deformações principais, foi possível concluir que o comportamento biomecânico com a inserção da TPC se revelou mais favorável na fase inicial do processo de cicatrização óssea. Ao invés, nos estágios finais do processo de cicatrização, fase II e III, o cenário inverte-se e o comportamento mais favorável verifica-se com a inserção do DHS.
With advancing age, the elderly population is increasingly susceptible to bone fractures, due to the loss of bone mass that comes from pathologies usually associated with this age group. Fractures of the proximal femur, particularly trochanteric fractures, are the most common fractures, while trochanteric fractures with reverse obliquity constitute a greater challenge for the orthopaedic surgeon. In this context, the main method of treatment uses internal bone fixation systems. However, the fact that existing systems on the market do not prove to be 100% efficient, with the need for its replacement during the life of the patient, leads to the development of new solutions.The main objective of this dissertation is to compare the biomechanical behaviour of a new Trochanteric Plate of Contention / Compression (TPC) with a Dynamic Hip Screw (DHS), used in large scale in the referred context. The numerical evaluation of the plates involved different phases of the bone regeneration process of a trochanteric fracture with reverse obliquity: immediately after the insertion of the systems (I), partial recovery of the fracture (II) and total recovery of the lesion (III). In this study, the influence of the abductor muscles in the loading conditions of the sets was also analysed.The numerical simulations performed throughout this work were made using the finite element calculation software, ADINA®, based on a set consisting of a digital femur model, the Sawbones® brand (CAD file # 3403), as well as 3D models of the plates, TPC and DHS, previously developed through CAD and CAE modelling systems.From the analysis of the results obtained for the magnitude of the displacements, as well as for the distribution of the von Mises stress and the principal strains, it was possible to conclude that the biomechanical behaviour with the insertion of the TPC was more favourable in the initial phase of healing process. Instead, in the final stages of the healing process, phase II and III, the scenario reverses and the more favourable behaviour occurs with the insertion of DHS.
