Title: A diabetes mellitus tipo 2 nos cuidados de saúde primários : o papel da insulinoterapia

Abstract: Introdução: A Diabetes mellitus constitui um grave problema de saúde, e em Portugal estima-se que existam 11,7% de diabéticos. Entre as opções farmacológicas de que se dispõe para o tratamento da Diabetes mellitus tipo 2 encontram-se os antidiabéticos orais e a insulina. A maioria dos doentes, devido à evolução natural da doença, acaba por necessitar de insulinoterapia que pode ser realizada em associação com medicação oral ou em monoterapia. No entanto existe, habitualmente, uma grande resistência ao inicio da insulinoterapia, quer por parte dos doentes, quer dos seus médicos assistentes, o que leva a um início tardio da insulinização. Objetivos: Rever a evidência atual sobre a insulinoterapia em diabéticos tipo 2 seguidos em Cuidados de Saúde Primários e os esquemas terapêuticos existentes, assim como abordar as crenças que levam a um atraso da introdução da insulinoterapia. Desenvolvimento: Existem diferentes tipos de insulinas classificadas de acordo com a sua origem e tempos de ação, assim como diferentes esquemas terapêuticos de insulina em associação com antidiabéticos orais e insulina em monoterapia. É notória a resistência ao início da insulinoterapia, quer por parte dos doentes, quer dos seus médicos assistentes. O medo das injeções, do aumento de peso e de restrições na qualidade de vida, são alguns dos obstáculos à insulinização, o que se reflete num atraso do controle efetivo da doença, com as complicações que daí advêm. Para quebrar estes obstáculos é importante criar normas claras de tratamento com insulina e sensibilizar tanto médicos como doentes das vantagens deste tratamento. Conclusão: Existem vários esquemas terapêuticos para início e titulação da insulina, pelo que cabe ao médico assistente adotar na sua prática clínica o que melhor se adequa ao seu doente, de acordo com as normas vigentes. As crenças que médicos e doentes possuem sobre esta terapêutica devem ser combatidas, de forma a aumentar a taxa de insulinização
Introduction: Diabetes mellitus is a major health problem and in Portugal there are about 11,7% diabetic patients. Among the pharmacological options available for the treatment of type 2 diabetes mellitus are oral antidiabetic agents and insulin. The majority of patients, due to the natural progression of the disease, eventually require insulin therapy that can be performed in association with oral medication or as monotherapy. However there is usually a great resistance to the initiation of insulin therapy, either by patients or their physicians, leading to a delayed onset of insulinization. Objectives: To review the current evidence on the insulin in type 2 diabetics followed in Primary Health Care and the existing treatment regimens, as well as address the beliefs that lead to a delay of the introduction of insulin therapy. Development: There are different types of insulin products classified according to their origin and time of action, as well as different insulin regimens in combination with oral antidiabetic agents and insulin monotherapy. There is notable resistance to the initiation of insulin therapy, either by patients or their physicians. Fear of injections, weight gain and restrictions on quality of life, are some of the obstacles to insulinization, which is reflected in a delay of effective control of the disease, with complications resulting therefrom. To break these barriers is important to establish clear standards of treatment with insulin and sensitize both doctors and patients of the advantages of this treatment. Conclusion: There are several therapeutic regimens for initiation and titration of insulin, so it is up to the physician to adopt the one that best fits his patient according to current regulations. The beliefs that doctors and patients have about this therapy should be tackled in order to increase the rate of insulinization.
