dc.description.abstract | O síndrome hepatorrenal (SHR) caracteriza-se por uma insuficiência renal em doentes com hepatopatia e tem uma elevada taxa de mortalidade. Este trabalho consiste num artigo de revisão sobre o SHR e foi feito com base numa pesquisa bibliográfica da pubmed. O objetivo deste trabalho foi contextualizar o conhecimento já existente sobre este síndrome. Para tal, foram sumariadas as várias descobertas nesta área ao longo dos últimos anos relativas à incidência e prevalência, variantes do SHR, novos marcadores de insuficiência renal, fisiopatologia, fatores precipitantes, estratégias preventivas, clínica, diagnóstico, diagnóstico diferencial e tratamento. A fisiopatologia do SHR envolve vários sistemas orgânicos e é importante na compreensão do mecanismo de ação da doença e no desenvolvimento de estratégias terapêuticas. Os fatores precipitantes do SHR devem ser evitados e devem ser tomadas medidas profiláticas e uma vigilância rigorosa da patologia hepática de base. A terapêutica definitiva é o transplante hepático. Porém, já existem tratamentos promissores, como os fármacos vasoconstritores, o TIPS e as terapêuticas de reposição renal e hepática. | pt |
dc.description.abstract | Hepatorenal syndrome (HRS) is a form of renal failure with high mortality that occurs in patients with advanced liver disease. In this review, based in published data from pubmed in the previous years, several aspects of this syndrome will be focused, like its incidence and prevalence, types of HRS, new markers of renal failure, physiopathology, precipitating factors, prevention, clinical aspects, diagnosis, differential diagnosis and treatment. The underlying physiopathological mechanisms of HRS involve multiple organic systems and their comprehension is important to the development of new therapeutic strategies. There are several precipitating factors that should be avoided and profilatic measures that should be taken, as well as a rigorous control of the underlying hepatic disease. Hepatic transplantation is the only definitive treatment. Despite of that, there are new promising therapies like vasoconstrictors, TIPS and hepatic and renal supportive treatments | pt |
