“(Poli)medicação e desempenho em testes psicométricos no adulto idoso diagnosticado com depressão”

Abstract

A farmacoterapia no doente idoso é uma temática complexa e que requer cuidados acrescidos tendo em conta as alterações fisiológicas e as co-morbilidades que caracterizam esta faixa etária da população. Associado a este fato existe também o aumento da ocorrência dos acontecimentos adversos, definidos como qualquer manifestação clínica desfavorável, independentemente da existência de relação causal com a intervenção.Não se pode excluir à partida o efeito dos fármacos sobre o funcionamento mental dos participantes uma vez que existe potencial para propiciarem o aparecimento de acontecimentos adversos no SNC, nomeadamente depressão e sintomatologia ansiosa. Os instrumentos psicométricos, realizados nestas condições, poderão ter os resultados enviesados por estes efeitos e não traduzir as capacidades cognitivas e o funcionamento emocional da pessoa examinada. O Patient Health Questionnaire-9 (PHQ-9) é um dos questionários utilizados para rastreio da depressão em ambiente clínico e esteve na base do desenvolvimento da proposta de consulta farmacêutica.O objetivo desta consulta farmacêutica será aplicar a mesma aos doentes, em ambiente de farmácia comunitária, antes e após o início da terapêutica com os fármacos identificados como tendo potencial para induzir ou agravar os sintomas depressivos/de ansiedade. Com este método pretende-se detetar o aparecimento de reações adversas medicamentosas, nomeadamente ao nível do SNC, minimizando o seu desenvolvimento e promovendo, acima de tudo o bem-estar e a segurança do doente.

Pharmacotherapy in the elderly patient is a complex subject and requires increased care due to the physiological changes and comorbidities that characterize this age group. Associated with this fact, there is also an increase in the occurrence of adverse events, defined as any unfavorable clinical manifestation, regardless of the existence of a causal relationship with the intervention.The effect of the drugs on the participants' health status cannot be ruled out at the onset, since there is potential to induce the manifestation of CNS adverse events, namely depression and anxious symptomatology. Psychometric instruments, performed under these conditions, may have the results skewed by these effects and they may not show the actual state of the analyzed person.The Patient Health Questionnaire-9 (PHQ-9 is one of the questionnaires used to screen depression in clinical settings and was the basis of the development of the pharmaceutical consultation proposal.The purpose of this pharmacist consultation will be to apply it to patients in a community pharmacy setting before and after starting therapy with drugs identified as having the potential to induce or aggravate depressive / anxiety symptoms. This method is intended to detect the appearance of adverse drug reactions, namely at the CNS level, minimizing its development and promoting above all the patient's well-being and safety.
