dc.description.abstract | A doença de Alzheimer é uma doença neurodegenerativa, progressiva, multifatorial, incurávelconsiderada a causa mais comum de demência e com prognóstico desfavorável. É caracterizada por um desequilíbrio entre a quantidade de proteínas β-amiloídes e proteínas TAU produzidas e eliminadas. O tratamento atual da doença é essencialmente sintomático, usando inibidores da colinesterase e Memantina, atuando sobre o sistema glutaminérgico. Nenhum medicamento novo foi aprovado para o tratamento da doença de Alzheimer desde 2003. O diagnóstico da doença é feito usando muitos testes cognitivos, como o Mini Mental State Examination, e técnicas de imagem, como a tomografia por emissão de positrões, entre outras, permitindo um diagnóstico mais correto, uma vez que os os sintomas da doença são muito similares aos apresentados noutras formas de demência.Novas formas de tratamento são necessárias numa população crescente de pacientes idosos, de indivíduos geneticamente predispostos (portadores do gene APOEε4) e de indivíduos assintomáticos que estão em risco de desenvolver a doença.Nos últimos anos, o uso de imunoterapia, tanto ativa, como passiva, usando anticorpos anti-β-amilóides, tem sido estudado e explorado para o tratamento da doença de Alzheimer. Os objetivos deste trabalho são: apresentar a situação atual do uso terapêutico dos anticorpos monoclonais contra as proteínas β-amilóides (IVIG, Bapineuzumab, Solanezumab, Gantenerumab, Ponezumab, Crenezumab, BAN-2401,Aducanumab, GSK933776, SAR-228810, implante secretor de anticorpos), que têm sido desenvolvidos para o tratamento de da doença de Alzheimer; identificar alguns dos desafios atuais e as futuras direções no tratamento e prevenção da doença de Alzheimer. | por |
dc.description.abstract | Alzheimer’s disease is an incurable multifactorial progressive neurodegenerativedisorder considered the most common cause of dementia and with a poor prognosis. It ischaracterized by an unbalance between the amount of amyloid-β proteins and TAU proteinsproduced and their clearance. The current treatment of the disease is essentiallysymptomatic, involving cholinesterase inhibitors and memantine, acting on the glutamatergicsystem. No new drugs were approved for the treatment of Alzheimer disease since 2003. AD diagnosis is made using many cognitive tests, as the Mini Mental State Examination and imagingtechniques, like positron emission tomography scans, among others, allowing a moreaccurate diagnosis once the symptoms are very similar to other forms of dementia. Newtherapies are necessary in a growing population of elderly patients, genetically predisposedindividuals (APOE ε4 carriers) and asymptomatic individuals who are at risk for Alzheimer disease. In the last years immunotherapies, both active and passive anti-β-amyloid antibodies, have beenexplored for the treatment of Alzheimer disease. The aims of this essay are to present the current status of the therapeutic use of monoclonal antibodies against amyloid beta proteins (IVIG,Bapineuzumab, Solanezumab, Gantenerumab, Ponezumab, Crenezumab, BAN-2401,Aducanumab, GSK933776, SAR-228810, antibody-secreting implant) that have been used for the treatment of Alzheimer disease, and to identify some of the current challenges and future directions in Alzheimer disease treatment and prevention. | eng |
